内容記述 | Epidemiological studies of biliary tract cancer (BTC) have been conducted over the last decade, the rationale being that the death rates in Niigata Prefecture where we live were found to be the highest in Japan for this cancer. The present paper proposes a hypothesis for seeking a determinant factor(s) and test this by conducting analytical studies. Since it was suggested that the prefectures in Japan and the cities and counties in Niigata where the mortalities from BTC were higher corresponded with rice producing areas, we began to search for a link between rice production and BTC mortality. Environmental factors taken into consideration included agricultural chemicals, dietary patterns which may be specific to rice producing areas and geological characteristics of the soil and drinking water. None of these factors, however, seems to have acted as a single cause in the etiology of BTC. On the basis of our analytical studies, we generated a multifactorial causation hypothesis: those who have a genetic susceptibility and a past history of gallstones or cholecystitis risk suffering from BTC (female gallbladder cancer in particular) when they are exposed to geographically specific environmental factors such as agricultural chemicals (e.g. diphenylether herbicides and their metabolites) in paddy fields. Based on the gas-chromatographic analysis of tap water, the water-borne route via the contaminated Shinano and Agano rivers is the most suspicious. | |||||
