内容記述 | 多血小板血漿と吸収性人工骨移植材である多孔性ハイドロキシアパタイト顆粒を混合した移植材を作製し、歯周骨内欠損に対する臨床効果を検討した。慢性歯周炎と診断され歯周基本治療後に少なくとも1カ所以上の部位に6mm以上の歯周ポケット深さ(PPD)とアタッチメントロス(CAL)を有し、かつエックス線写真で3mm以上の骨内欠損が認められた20名20部位を対象とした。全層歯肉弁を形成・剥離して病巣部掻爬完了後にアルギン酸ナトリウムで活性化した多血小板血漿(PRP)と多孔性ハイドロキシアパタイト顆粒(HA)の混合移植材(PRP+HA)を填入し緊密に縫合した。術後1年日に臨床評価およびエックス線写真による評価を行いベースラインの値と比較検討した。gingival index(GI)およびbleeding on probing(BOP)はベースラインと比較して1年目において有意に低かった(GI:0.85±0.67→0.10±0.31、P<0.01、BOP:0.95±0.22→0.15±0.37、P<O.01)。 pocket probing depth(PPD)およびclinical attachment level(CAL)はベースラインと比較して1年目において有意に低かった(PPD:7.35±1.87mm→3.15±0.88mm、p<0.001、CAL:7.75±1.89mm→4.85±1.50mm、p<0.001)。エックス線的骨欠損深さはベースラインと比較して1年目において有意に浅かった(6.40±0.88mm→2.65±0.99mm、p<0.0001)。以上より多血小板血漿と多孔性ハイドロキシアパタイト顆粒の混合移植材は術後1年の時点で臨床的に有効に作用したことが明らかとなった。 | |||||
