This study was performed to clarify the clinical efficacy of the flow cytometry method for detecting ANCA in the serum of patients with UC, comparing this with an indirect immunofluorescence microscopy (IIF). Materials and Methods : Forty-two sera from 21 patients with active and inactive UC and seven sera from 7 patients with CD were obtained. Flow cytometry and IIF methods were employed to detect ANCA. For flow cytometry, indicator neutrophils were permeabilized by Saponin treatment, resuspended in the diluted sera, and incubated in FITC-conjugated goat antihuman immunoglobulin. These cells were analysed on a flow cytometer. For IIF, a commercial kit to detect ANCA was used. Results : Flow cytometry showed ANCA positivity in thirty-five of the 42 sera tested, and IIF in twenty-eight of the 42 sera tested. When compared with IIF, the flow cytometry for ANCA had a sensitivity of 96.5% and a specificity of 63.6%. ANCA positivity by flow cytometry and by IIF appeared in 23 and 19 of 28 sera tested in the entire colitis, respectively, and yielded almost similar results in left sided colitis. Positive rates and titers for ANCA by flow cytometry in the chronic continuous and relapse/remittent types of UC (intractable type) were significantly higher than those of the initial attack type of UC (non-intractable) and CD. Conclusion : ANCA positivity by flow cytometry was higher than that by IIF.
