| 内容記述 | Plunging ranula is a form of retention cyst occurring in the submandibular region. In this paper, a case of plunging ranula which did ot respond to the compression therapy is reported. A 38-year-Old woman visited us because of a fluctuant swelling of the right ubmandibular region. A cranial radiograph with injection of 76% Urografin revealed the presence of a cystic lesion in the submandibular region without extension to the sublingual area. This was further confirmed by MRI. The clinical diagnosis was lunging ranula. Following removal of the cystic fluid by aspiration, the cystic area was compressed by gauze, but the lesion recurred in a onth. The compression therapy was repeated again, but proved to be unsuccessful. Thus, extirpation of the sublingual gland and arsupialization of the ranula were performed. Postoperatively, there has been no evidence of recurrence of the lesion for 4 years. icroscopically, there was chronic adenitis of the sublingual gland. | |||||

| 内容記述 | 顎下型ガマ腫は顎下部に生ずる粘液嚢胞で、その報告は比較的少ない｡今回、私たちは顎下型ガマ腫の1例を経験したので、その概要を報告する｡症例は38歳の女性で、平成4年7月30日、右顎下部の腫脹を主訴として当科を受診した｡全身的所見では問題なく、口腔外所見では右顎下部に腫脹を認め波動を触れた｡口腔内所見では右口底部に軽度腫脹を認めた｡造影Ⅹ線所見、MRI所見ともに右顎下部に嚢胞様病変を認めた｡処置および経過:右顎下型ガマ腫の診断にて吸引･圧迫療法を2度行うも再発したため、平成4年12月17日、右舌下腺摘出術、ガマ腫開窓術を施行した｡術後3年8か月を経過したが再発は認めず経過良好である。病理組織学的所見では、舌下腺に慢性の唾液腺炎を生じていた｡ | |||||
