| 内容記述 | This study aims at clarification of the mode of progressive destruction of bony elements of RA-elbow joint and the time taken for progress from a certain stage to a more advanced stage. Methods: 1. The author classified 142 RA-afflicted elbow joints of 77 cases into 6 stages based on severity of bony destruction on X-ray. Radiological indicators representing severity of bony destruction were established on humeral, radial and ulnar component of an elbow joint. Average value of each indicator was calculated in each stage. Based on these data, the mode of progressive destruction of bony elements of RA-elbow joint was estimated. 2. In 50 non-operated RA-elbow joints of 33 cases and 23 synovectomized RA-elbow joints of 21 cases which have been followed-up by X-ray taken repeatedly more than 3 times, the mode of progressive bony destruction and the time taken for stage-progression were investigated. Results: The cortical thinning of the distal shaft of humerus, which appeares prior to articular destruction, progresses in accordance with the progressive articular destruction. Articular destruction begins at and progresses from trochlea of the humerus condyle, from the center of and the coronoid process of the trochlear notch and from the proximo-ulnar site of radius head. Articular destruction begins at and advanced from humero-ulnar joint in 78% of RA-afflicted elbow joint. In the majority of non-operated joints, progression from stage 1 to 5 took 4-6 years. Progression from stage 5 to 6, which took more time than for progression in the lower stages was observed in 9 joints out of 42 which advanced up to the stage 5. No significant difference in the time for stage progression was noted between non-operated and synovectomized RA-afflicted elbow joints. | |||||
