| 内容記述 | We report a case of severe trismus due to cicatricial contracture along the masticatory muscle after temporal phlegmon was improved. A 62-year-old male visited our hospital with the severe trismus after temporal phlegmon. The trismus was persistent despite continuous mouth-opening training for several months. Clinical characteristics included a "hard-end feeling" at the time of maximum mouth opening, and a forward and lateral sliding movement of the condylar head, as seen in the case of masticatory muscle tendon–aponeurosis hyperplasia. Magnetic resonance imaging showed cicatricial tissue along the temporal and masseter muscles from the cranium to the coronoid process of the mandible, but there were no abnormal findings around the temporomandibular joint. The persistent trismus was improved by surgically removing the coronoid process along with cicatricial tissue and by an earlier initiation of continuous mouth-opening training postoperatively. | |||||||||||||||||||||||||

| 内容記述 | われわれは，側頭部蜂窩織炎後に残遺した瘢痕による開口障害の1例を報告する。顎顔面口腔領域における炎症性開口障害は，消炎に伴い自ずと改善されることがほとんどであるが，ごくまれに炎症が長期に及び開口障害が持続することがある。患者は62歳の男性で，側頭部蜂窩織炎の消炎後に開口障害が改善せず，当院を受診した。開口障害の治療として，まず保存的に開口訓練を数か月間行ったが，改善が得られなかった。保存療法での改善が認められない場合，外科療法を考慮するが，その明確な適応基準はない。本例の臨床的特徴として，end feelの硬い開口障害で，側頭筋腱・腱膜過形成症と同様に前方運動や側方運動などの下顎頭の滑走が認められた。画像診断では，MRI画像にて，側頭筋，咬筋に沿って頭蓋底から筋突起におよぶ瘢痕が帯状に形成されており，顎関節部には異常所見はなかった。瘢痕および筋突起の切除により術中開口量は十分に回復し，術後早期からの開口練習により症状は改善した。 | |||||||||||||||||||||||||
