| 内容記述 | Adequate selection of supportive method during surgery for ruptured aortic aneurysms is important because it affects the surgical outcome. Surgical outcome was reviewed with this in mind in 12 patients who had undergone surgery in the past decade. The mean age of the patients was 72.2 years. The site of the aneurysms was the ascending aorta in one patient, the aortic arch in 6 patients, and the descending aorta in 5 patients. The rupture occurred in pericardial cavity in 2 patients, the mediastinal space in 6 patients, and the left pleural cavity in 4 patients. Nine patients (75%) developed shock. Surgery was performed within 28 hours after the occurrence of the rupture in 10 patients (83.3%). The Patient with the aneurysm of the ascending aorta underwent graft replacement while maintaining complete extracorporeal circulation. Five of the 6 patients with aortic arch aneurysms underwent graft replaccment of the arch with cardiopulmonary bypass combined with selective cerebral perfusion, and the other patient underwent surgery by the patch aortoplasty. Among the 5 patients with aneurysms of the descending aorta, selective perfusion of the upper and lower half of the body was performed in 1, partial cardiopulmonary bypass in 3, and temporary bypass in 1. Two of the 6 aortic arch aneurysm patients (33.3%) developed serious postoperative complications. The patient with old myocardial infarction died complicating acute cardiac failure and cerebral damage. The patient in whom respiratory arrest had occurred preoperatively developed acute cardiac failure and died. Postoperative complications occurred in 4 (80%) of the 5 patients in whom graft replacement was performed for descending aortic aneurysms. The patient who had experienced cardiac arrest immediately before the operation died because of inability to wean the patient from extracorporeal circulation. The patient with coexisting dilated cardiomyopathy developed acute heart failure and respiratory failure and ultimately died. Of the 2 patients with intraoperative bleeding, one developed cerebral infarction and acute renal failure, and the other developed paraplegia. The operative mortality was 33.3%, and the hospital mortality was 16.7%. | |||||
