| 内容記述 | わが国で発生する原発性肝癌のうち最も頻度の高いものはC型肝炎ウイルス(HCV)の持続感染が原因で生じる肝細胞癌である.HCVの持続感染によってATL値の高値が持続すると徐々に肝線維化が進行し肝癌の発生率が上昇する.HCV感染の排除にはインターフェロン治療が有効である.インターフェロン治療による治癒しやすさを予測する因子が明らかになっており,ゲノタイプ1b,高ウイルス量の症例に対しての著効率は低い.インターフェロン治療によって治癒した症例やATL値の正常化の得られた症例は肝癌の発生率が減少する.しかしながらインターフェロン治療には副作用も多く,高齢者や合併症の存在する例には治療を行い得ないことも多い.近年リバビリンという経口の抗ウイルス薬をインターフェロンと併用する治療が可能になり,難治性のC型肝炎の著効率は上昇した.またポリエチレングリコール(PEG卜インターフェロンの出現により副作用の少ない遂行率の高い治療法が標準治療となり,C型肝炎撲滅に向けた新時代が到来している. | |||||
