認知症の長期ケアにおける人権擁護といった視点で重要な課題である. そこで, 本研究では, わが国における認知症高齢者の自己決定に関する研究の動向を探ることを目的とし, 1983年以降から2007年に至るまでの約24年間の文献の検討をすることにした. 【方法】文献の検索には, 国内医学文献情報データべースである医学中央雑誌のWeb版 (医中誌Web) Ver.4を用いた. キーワードを｢認知症｣｢権利擁護｣｢意思決定｣｢代理判断｣｢成年後見制度｣｢同意能力｣とし, 該当した文献は237件であった. 認知症高齢者の自己決定に関する文献237件について, 5年毎に区切り年代別にその推移を把握するとともに, 医学中央雑誌のWeb版の論文種類の区分である原著論文, 解説, 総説, 会議録, 一般を用いて論文の種類別件数についてまとめた. 原著論文の34件に対しては, 論文内容を精読し, その記述内容について比較検討した上で, 研究の主題を分類した. 【結果】文献数の推移としては, 2000年以降に認知症高齢者の自己決定に関する文献が公表されるようになり, 2003年頃よりその数は急増した. 論文種類別件数では, 原著論文34編, 解説140編, 総説1編, 会議録50編, 一般12編であった. 原著論文のうち認知症高齢者の自己決定に関わるものではない6編を除いた28編の主題を分析したところ, ｢患者の自己決定に関わる支援｣5編, ｢特産の状況の援助の中において付随する認知症高齢者の自己決定｣4編, ｢認知症や自己決定に関する意識｣6編, ｢高齢者の自己決定と医療の現状｣4編, ｢制度の活用｣9編に分類された. 【考察】認知症高齢者の自己決定に関する文献検討を行ったところ, 2000年以降よりようやく文献が公表されるようになり, 2003年頃よりその数は急増したが原著論文の数はさほど多くはない. 論文種類の中でも, 患者などのデータに基づかない専門委員会や尊門家個人の意見である解説の割合は高い. その一因には, 認知症高齢者を研究対象とすることの困難さが挙げられよう. 検討結果から, 認知症高齢者の自己決定をめぐる医療の課題として, 自己決定能力のアセスメントや, 能力評価を左右する要因, 自己決定を支える看護師を含めた専門職の役割, 成年後見制度などの制度の有効な活用等について,
