[{"subitem_description": "顔面骨骨折は, 形成外科ではよく経験する症例のひとつであるが, 施設により診療科が異なる. 新潟市民病院では, 当科以外にも, 救命救急科, 耳鼻咽喉科, 眼科, 口腔外科, 脳神経外科で分担, 連携し診療を行っている. 今回われわれは, 顔面骨骨折の近年の動向を示すため, 当院のこれらの科を受診した顔面骨骨折症例について統計をとった. 対象は, 2007年11月から2010年10月までの3年間に当院を受診した新鮮顔面骨骨折症例428例500骨折とした. 428例中男性315例, 女性113例で, 男女比は2.8:1であった. 各年の症例数に大きな差はなく, 月別症例数でも明らかな共通点は認められなかったが, 5月に交通事故が多い傾向があった.男女とも10代が最多であったが, 高齢層の割合も比較的高く, 特に女性は70代にもピークを認め, 高齢層での男女比はほぼ等しかった. 受傷原因は, 不慮の事故32.0%, 交通事故30.0%が同程度に多く, 次いでスポーツ20.8%であった. 交通事故は全年代に分布していた. 不慮の事故も全年代に分布していたが, 10歳未満と高齢層での割合が高かった. スポーツは90%以上が10～30代であり, 特に10代では受傷原因の約半数を占めた. 受傷部位は鼻骨が39.6%と圧倒的多数であり, なかでも男性の鼻骨骨折は全骨折の30.8%を占めていた. 鼻骨以下は頬骨25%, 眼窩14.2%, 下顎骨10.2%と続いた. 鼻骨骨折の原因はスポーツが最多であったが, 他の受傷部位は交通事故または不慮の事故が最多であった. 頬骨骨折は交通事故において鼻骨骨折より多く, スポーツ, 暴力行為において, 眼窩骨折より少なかった. 整復手術を施行したのは163例(38.1%), 185骨折 (37.0%) であった. 受傷部位別の手術施行率は前頭骨14.3%, 眼窩33.8%, 頬骨31.2%, 鼻骨42.9%, 上顎骨33.3%, 下顎骨47.1%であり, 下顎骨骨折ではほぼ半数が手術を必要とした. 従来の報告と同様な統計結果が得られたが, 当院では, 高齢層, 特に高齢女性の割合が高かったこと, 自転車事故が自動車事故と同等に多かったこと,
