{"created":"2021-03-01T06:12:55.174315+00:00","id":8876,"links":{},"metadata":{"_buckets":{"deposit":"2b40879f-51d6-4dfc-8cd2-e7a0bf43d6d6"},"_deposit":{"id":"8876","owners":[],"pid":{"revision_id":0,"type":"depid","value":"8876"},"status":"published"},"_oai":{"id":"oai:niigata-u.repo.nii.ac.jp:00008876","sets":["453:456","471:537:538:540"]},"item_7_alternative_title_1":{"attribute_name":"その他のタイトル","attribute_value_mlt":[{"subitem_alternative_title":"Surgical Treatment for Breast Cancer"}]},"item_7_biblio_info_6":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2014-08","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"8","bibliographicPageEnd":"351","bibliographicPageStart":"343","bibliographicVolumeNumber":"128","bibliographic_titles":[{"bibliographic_title":"新潟医学会雑誌"},{"bibliographic_title":"新潟医学会雑誌","bibliographic_titleLang":"en"}]}]},"item_7_description_4":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"乳癌は従来, 欧米に比べ日本人では少ないとされていた. しかし近年では罹患率は急速に増加しており, 最新データでは日本人女性の乳癌の生涯リスクは7% (14人に1人)となっている. 乳癌の治療は, 大きく外科的治療(手術療法), 薬物療法, 放射線療法の3つに分けられるが, 症例ごとに乳癌の病期, 性質を考慮してこれらの治療法を組み合わせていく, いわゆる集学的治療が重要である. 今回は特に乳癌の外科的治療に焦点を絞って解説する. 乳癌に対する外科治療の歴史は正確にはわからないが, 紀元前1650年頃のパピルスに記録として残っている最古のものと考えられており, 乳房の腫瘍性病変を切開して排膿し残存腫瘍を焼却・腐敗させた内容が記されている. 古代から前近代においては乳癌の治療は焼却・腐食・切除が行われていたとされるが, 根治的効果は乏しく, 乳癌治療が発展するには近代まで待たなければならなかった. 17世紀に入ると癌腫を周囲組織を大きく付けて取ることと, 腫大した腋窩リンパ節を取ることが乳癌手術手技として確立された. しかし今日の乳房切除とは異なり不完全な切除であった. 19世紀に入ると麻酔法の発展を背景に, 乳癌の手術術式は腫瘍切除から乳房切除へと移っていき, さらに広範な皮膚切除や大胸筋筋膜切除も行われるようになった. しかし術後の再発, 特に局所再発の頻度は極めて高く, 高名な外科医でも乳癌術後の局所再発率は50～80%程度であった. その後, William Stewart Halstedは乳癌の再発は不完全な切除が主な原因と考え, 乳癌を完全に取り除くために乳房, 大胸筋, 小胸筋を一塊に切除し, さらにリンパ節を含め脂肪織ごと郭清を行うen bloc郭清を考案した. 彼の術式は定型的乳房切除術(radical mastectomy)と呼ばれ, 3年生存率42.3%, 局所再発率6%と, 当時としては驚異的に良好な成績を示したため1980年代後半まで日本でも広く行われていた. その後, 大胸筋を温存して乳房切除および腋窩リンパ節郭清を行う非定型乳房切除術が考案され, 治療成績は定型的乳房切除術手術と同等以上であるため現在でも行われている. さらに1943年には乳房部分切除でも適切な放射線療法を行えば5年生存率88%と乳房切除術に匹敵することが示され, 放射線治療の進歩に伴い乳房温存手術は全世界的に普及し, 現在, 我が国でも約6割の症例で乳房温存手術が行われている. また, 腋窩リンパ節郭清はリンパ浮腫・上腕の知覚鈍麻といった合併症リスクを伴う手技であるため, 近年はセンチネルリンパ節生検(sentinel lymph node biopsy: SLNB)という手技が確立し, 乳癌手術では標準術式となっている.","subitem_description_type":"Abstract"}]},"item_7_full_name_3":{"attribute_name":"著者別名","attribute_value_mlt":[{"nameIdentifiers":[{"nameIdentifier":"61871","nameIdentifierScheme":"WEKO"}],"names":[{"name":"Koyama, Yu"}]}]},"item_7_publisher_7":{"attribute_name":"出版者","attribute_value_mlt":[{"subitem_publisher":"新潟医学会"}]},"item_7_select_19":{"attribute_name":"著者版フラグ","attribute_value_mlt":[{"subitem_select_item":"publisher"}]},"item_7_source_id_11":{"attribute_name":"書誌レコードID","attribute_value_mlt":[{"subitem_source_identifier":"AN00182415","subitem_source_identifier_type":"NCID"}]},"item_7_source_id_9":{"attribute_name":"ISSN","attribute_value_mlt":[{"subitem_source_identifier":"00290440","subitem_source_identifier_type":"ISSN"}]},"item_creator":{"attribute_name":"著者","attribute_type":"creator","attribute_value_mlt":[{"creatorNames":[{"creatorName":"小山, 諭"}],"nameIdentifiers":[{"nameIdentifier":"61870","nameIdentifierScheme":"WEKO"}]}]},"item_files":{"attribute_name":"ファイル情報","attribute_type":"file","attribute_value_mlt":[{"accessrole":"open_date","date":[{"dateType":"Available","dateValue":"2019-08-06"}],"displaytype":"detail","filename":"128(8)_343-351.pdf","filesize":[{"value":"1.2 MB"}],"format":"application/pdf","licensetype":"license_note","mimetype":"application/pdf","url":{"label":"128(8)_343-351.pdf","url":"https://niigata-u.repo.nii.ac.jp/record/8876/files/128(8)_343-351.pdf"},"version_id":"58b929f4-cdde-4887-b078-2e6923e9ea9c"}]},"item_keyword":{"attribute_name":"キーワード","attribute_value_mlt":[{"subitem_subject":"乳癌","subitem_subject_scheme":"Other"},{"subitem_subject":"乳房温存手術","subitem_subject_scheme":"Other"},{"subitem_subject":"局所再発","subitem_subject_scheme":"Other"},{"subitem_subject":"センチネルリンパ節生検","subitem_subject_scheme":"Other"}]},"item_language":{"attribute_name":"言語","attribute_value_mlt":[{"subitem_language":"jpn"}]},"item_resource_type":{"attribute_name":"資源タイプ","attribute_value_mlt":[{"resourcetype":"departmental bulletin paper","resourceuri":"http://purl.org/coar/resource_type/c_6501"}]},"item_title":"乳癌の手術療法","item_titles":{"attribute_name":"タイトル","attribute_value_mlt":[{"subitem_title":"乳癌の手術療法"},{"subitem_title":"乳癌の手術療法","subitem_title_language":"en"}]},"item_type_id":"7","owner":"1","path":["456","540"],"pubdate":{"attribute_name":"公開日","attribute_value":"2016-09-13"},"publish_date":"2016-09-13","publish_status":"0","recid":"8876","relation_version_is_last":true,"title":["乳癌の手術療法"],"weko_creator_id":"1","weko_shared_id":null},"updated":"2022-12-15T03:43:07.901771+00:00"}
