A 72-year-old man presented to our hospital for evaluation of a soft mass and pain in the right lateral abdomen. Physical examination revealed a soft, smooth-surfaced mass with a diameter of 8 cm. The mass disappeared when the patient was in the lateral supine position. A computed tomographic examination was performed under a tentative diagnosis of an external hernia. Computed tomography revealed an absence of the abdominal wall and the presence of prolapsed retroperitoneal fat under the 12th rib; these findings were compatible with a superior lumbar hernia. Endoscopy-assisted herniorrhaphy was performed via a subcutaneous approach. Dissection of the subcutaneous tissue, confirmation of the hernia orifice, and taxis of the hernia sac were carried out under endoscopy. Repair was performed using a Bard(R) Light PerFixTM Plug (Davol, Inc., a subsidiary of C.R. Bard, Inc., Warwick, RI, USA) with a 4-cm skin incision. The postoperative course was uneventful, and the patient was discharged on postoperative day 3.
