Glioblastomaは明らかな前駆病変なく,新規に急速な進行を示す原発性glioblastomaと先行する低悪性度星細胞腫から緩徐に進行し,悪性転化して発生する二次性glioblastomaに分けられる. Isocitrate dehydrogenase (IDH;イソクエン酸脱水素酵素)が両者を特徴づける重要な遺伝子変異であることが分かっており,原発性glioblastomaではIDH1/2の変異は極めてまれであり,二次性glioblastomaでは高頻度に認められる. Epidermal growth factor receptor (EGFR;上皮成長因子受容体)遺伝子増幅およびその変異型であるEpidermal growth factor receptor variant III (EGFRv III)はglioblastoma, IDH wild-typeに認められる代表的な遺伝子変異である. EGFRv IIIは腫瘍特異的な細胞膜抗原であり,リガンド非依存的に常時活性化し,細胞内シグナリングを増強し,血管増生と腫瘍増生を促進させることが知られている. EGFRv IIIはその腫瘍特異性から,これまで治療ターゲットとして注目を集めてきたが免疫組織化学的な検索が不十分であった.本研究では, Glioblastoma, IDH wild-typeと診断された48患者の手術標本を用いて,EGFRv IIIに特異的な抗体を用いて,免疫組織化学的検索を行い,細胞形態, EGFRv III陽性腫瘍細胞の分布を評価した.さらにはEGFRv III発現の予後への影響を評価した.EGFRv III陽性は再発1症例を含む12標本, 11患者で認められた.免疫染色では複数の突起を有するグリオーマ細胞においてEGFRv IIIおよびglial fibrillary acidic protein(GFAP)の発現が認められた.
