Specific pre-operative diagnosis of multilocular cyst obviates the need for extensive staging or radical surgery. Multilocular cyst of the kidney is an uncommon lesion with less than 200 reported in the literature. Until the advent of cross-sectional imaging, correct preoperative differentiation from a malignant tumor could not be made with any confidence.
