@misc{oai:niigata-u.repo.nii.ac.jp:00006080, author = {金澤, 勉}, month = {Mar}, note = {滑車神経はTranstentorial approach を行う外科的な操作において指標となり,テント切痕に近接して走行するため,テント切痕を切開するような場合は損傷されやすい.術前に画像上で滑車神経がテントに接触する場所を捉えておこくことは非常に重要である.また,滑車神経麻痺等の病態の解明の一助ともなりうるため滑車神経を画像で描出する意義は大きい.本研究は,three-dimensional constructive interference in steady state(3D-CISS)法による高空間分解能MRI を用いて滑車神経の同定に特化したmulti planar reconstruction(MPR)画像の作成を行い,滑車神経の同定に適する撮像条件について検討することを目的とした.脳神経の中で最も細い滑車神経の同定には非常に薄いスライス厚の設定が必須であり,0.5 mmおよび0.25 mmのスライス厚を用いた.このような薄いスライス厚の信頼性についての検討が必要であるが,現時点では1mm 未満の薄いスライス厚を測定できる方法は存在しない.そこで本研究では,従来のスライス厚測定法であるウェッジ法を改良することにより薄いスライス厚測定への適用を図った.第1章では,本研究の背景,意義および目的を述べた.高分解能MRI による中枢神経領域における脳槽内の微細な血管や神経などの画像化について,および従来の標準的なMR画像のスライス厚測定法について概説した.その後,本論文の構成について述べた.第2 章は,高分解能3D-CISS MRI により滑車神経の同定に適する撮像条件(主に磁場強度とスライス厚)について後ろ向き研究により検討することを目的とした.滑車神経の同定のための指導を受けた診療放射線技師が,滑車神経の同定に特化した断面のMPR 画像の作成を試みた.対象は異なる条件(1.5 T および3 T,スライス厚0.5 mm および0.25 mm)で撮像された総計40 症例の左右の滑車神経(80 本)である.滑車神経の同定率は,1.5 T 装置を用いた場合に比べ3 T の方が有意に高い結果であった.同定された滑車神経の距離を計測した結果,3 T ではスライス厚0.25 mm の方が0.5 mm 厚よりも有意に長くなり,またスライス厚0.5 mm では,1.5 Tよりも3 T の方が有意に長くなった.3D-CISS 法を用いて3 T 装置で,スライス厚0.25mmの設定による高分解能撮像を行い,滑車神経に特化した適切なMPR 画像を作成することにより,滑車神経の走行をより確実に,より長く同定できることが示唆された., 第3 章では,スライス厚測定法であるウェッジ法においてedge response function(ERF)の数値微分によるSNR の低下を防ぐために,非線形のフィッティングカーブをERF に適用した方法(改良ウェッジ法)を考案した.スライス厚5 mm と3 mm の測定において,改良ウェッジ法によって算出されたスライス厚は従来の標準的なスラブ法やウェッジ法による値とほぼ一致することから,改良ウェッジ法の妥当性が認められた.3D-CISS 法により撮像されたスライス厚0.5 mm と0.25 mm の画像のスライス厚測定において,ERF データへのカーブフィッティングは良好であり,得られたスライス厚は用いたスライス画像による変動は少なかった.改良ウェッジ法を用いて複数のスライス画像から得たスライス厚を平均することによって,1 mm 未満の薄いスライス厚についても信頼性の高い測定が可能であることが示された.本研究結果より,3D-CISS 法を用いてスライス厚を0.5 mm および0.25 mm に設定した場合,実効スライス厚はそれぞれ0.543 mm,0.247 mm となることが確認された.いずれも設定スライス厚の10%以下の差異であり,滑車神経の撮像条件として設定したスライス厚0.5 mmおよび0.25 mmの精度が高いことが示された.滑車神経の同定には,3D-CISS法を用いて高磁場(3 T)および高分解能(スライス厚0.25 mm)の条件で撮像を行い,滑車神経に特化した適切なMPR 画像を作成することにより,滑車神経の走行をより確実に,より長く同定できることが明らかとなった.ルーチン検査において滑車神経をより確実に同定するための一手法となるものと期待される., The trochlear nerve is the fourth cranial nerve and is the motor nerve of the superior oblique muscle of the eye. The trochlear nerve can easily be injured during surgical procedures. Preoperative identification of the trochlear nerve can assist in the preoperative planning. The trochlear nerve is so thin that it is difficult to identify reliably by Magnetic Resonance Imaging (MRI) and other imaging modalities in routine clinical practice. The objective of the present study was to evaluate the influence of higher magnetic field strength and higher spatial resolution for identifying the trochlear nerve with three-dimensional constructive interference of steady state (3D-CISS) MRI sequence. Very thin (sub-millimeter) slices were used for achieving higher spatial resolution. The accuracy of very thin slice thickness should be evaluated, however, there were no methods available for the measurements of sub-millimeter slice thicknesses. In the present study, the standard method (‘wedge method’) for measuring the slice thickness of MRI was improved so that it was applicable to the measurement of sub-millimeter slice thickness. This thesis comprises four chapters as follows. In Chapter 1: “Introduction” described the background and the objective of this study, and showed the outline of this thesis. In Chapter 2: “Evaluation of trochlear nerve with high-resolution 3D-CISS MRI” described the influence of higher magnetic field strength and higher spatial resolution for identifying the trochlear nerve with 3D-CISS sequence. The study was performed by retrospectively analyzing the MRI of 40 subjects (80 trochlear nerves); 28 trochlear nerves were examined in 1.5T MRI with slice thickness of 0.5 mm, 30 nerves in 3T MRI with slice thickness of 0.5 mm, and 22 nerves in 3T MRI with slice thickness of 0.25 mm. Two radiological technologists, who were specifically trained for the observation of the trochlear nerve on MRI, challenged to generate images by a multiplanar reconstruction (MPR) technique with adaptively varying the MPR parameters (e.g., oblique direction and slice location) according to the cisternal course of the trochlear nerve from the point of exit to the tentorial edge. The trochlear nerve identification was significantly better (p<0.05) on 3T MRI than on 1.5T MRI. The mean distance of the nerve measured on thin-slice (0.25 mm) 3T MRI was significantly longer (p<0.05) than that on 3T MRI (0.5-mm slice thickness), followed by that on 1.5T MRI. The proposed procedure for generating images by the MPR technique can be performed by trained radiological technologists. Use of high-resolution (0.25-mm slice thickness) 3D-CISS sequence on a 3T scanner accompanied with the proper MPR enables reliable identification of the trochlear nerve., In Chapter 3: “Measurement of sub-millimeter slice thicknesses” described the improvement of the wedge method for the measurement of sub-millimeter slice thicknesses in MRI. In the standard wedge method, the slice profile is obtained by numerical differentiation of the measured edge response function (ERF). However, differentiation of the ERF degrades the signal-to-noise ratio (SNR) of the slice profile, and therefore, it would be practically difficult to apply the method to measurements of very thin slices. The objective of this study was to improve the wedge method by fitting a curve to the ERF. The curve-fit function was obtained by convolving an ideal ERF (a ramp function) with a Gaussian function to represent ERF blurring. Measurements of 5- and 3-mm slice thicknesses were performed on a 3T scanner using the conventional wedge method, the improved wedge method, and another standard method using an inclined slab (‘slab method’). Subsequently, 0.5- and 0.25-mm slice thicknesses from multiple slices acquired using 3D-CISS sequence were measured using the improved wedge method. When measuring 5-mm slices, the differences in measurements obtained using the improved wedge method and the conventional slab and wedge methods were very small: <0.6% of the 5-mm slice thickness. The difference was ≤1.7% for 3-mm slices. For 0.5- and 0.25-mm slices, the mean values obtained using the improved wedge method were 0.543 ± 0.007 mm and 0.247 ± 0.015 mm, with a 1.2 and 5.9% coefficient of variation across slices, respectively. The validity of the improved wedge method was confirmed by the comparison with the standard methods, and potentially applicable to the measurement of sub-millimeter slice thicknesses. In Chapter 4: “Conclusions” summarized the results of this study. The accuracy of slice thickness of 3D-CISS MRI was verified by the improved wedge method.
