{"created":"2021-03-01T06:09:56.944208+00:00","id":6062,"links":{},"metadata":{"_buckets":{"deposit":"4e4bc87f-0af3-48bf-9e73-f21bc8791a3f"},"_deposit":{"id":"6062","owners":[],"pid":{"revision_id":0,"type":"depid","value":"6062"},"status":"published"},"_oai":{"id":"oai:niigata-u.repo.nii.ac.jp:00006062","sets":["432:433:434","453:455"]},"item_6_biblio_info_6":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2018-03-23","bibliographicIssueDateType":"Issued"},"bibliographicPageEnd":"32","bibliographicPageStart":"1","bibliographic_titles":[{}]}]},"item_6_date_granted_51":{"attribute_name":"学位授与年月日","attribute_value_mlt":[{"subitem_dategranted":"2018-03-23"}]},"item_6_degree_grantor_49":{"attribute_name":"学位授与機関","attribute_value_mlt":[{"subitem_degreegrantor":[{"subitem_degreegrantor_name":"新潟大学"}]}]},"item_6_degree_name_48":{"attribute_name":"学位名","attribute_value_mlt":[{"subitem_degreename":"博士（歯学）"}]},"item_6_description_4":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"超高齢社会では，高齢者の疾病予防や健康増進，介護予防などの取り組みが必要であり，いかに健康増進とQOLの向上を図るかが，医療のみならず社会的にも経済的にも重要である．しかし，残存歯数や義歯の状態とQOLとの関連は示されているものの，舌や口唇の動きといった口腔機能とQOLとの関係を評価した報告はあまり見られない．そこで，本研究では，介護予防事業に参加する自立高齢者を対象として，口腔関連QOLと舌や口唇の動きを含めた口腔機能との関連性を検討するとともに，同事業で用いられる口腔機能向上プログラムの効果を検証した．対象者は，台湾の介護予防教室に参加している自立高齢者165名（男性43名，女性122名，平均年齢74.8±7.1歳）とした．口腔関連QOLの評価法としてGOHAIを使用し，口腔機能評価項目として，舌左右運動の速さ，最大舌圧，最大口唇圧，反復唾液嚥下テスト，オーラルディアドコキネシス，咀嚼能力（グミゼリーを用いた咀嚼能率スコア）を測定した．さらに残存歯数と義歯使用の有無，ならびに食事時に自覚している問題点について調査した．分析は，それぞれの項目ごとにカットオフ値を設けて2群に分け，各群間におけるGOHAIスコアを比較した．また，GOHAIスコアの第1四分位を基準として2群に分けて目的変数とし，単変量解析で有意差のあった口腔機能項目を説明変数としてロジスティック回帰分析を行った．単変量解析の結果，義歯使用の有無，アイヒナー分類，舌左右運動の速さ，オーラルディアドコキネシス/ta/，/ka/，咀嚼能力の各項目でGOHAIスコアには有意な差が認められた．また，ロジスティック回帰分析の結果，オーラルディアドコキネシス/ka/（odds=3.524）がGOHAIスコア低値の有意な説明変数として選択された．以上より，口腔関連QOLの低下と舌の後方の運動機能の低下との関連性が示唆された．次に，1回目の165名のうち98名（男性27名，女性71名，平均年齢75.8±6.9歳，60-91歳）に対して，自宅で行う口腔機能向上プログラムを指導し，12週後に再度口腔機能測定を行った．口腔機能向上プログラムとして(1)下で左右の頬を5回ずつ押し出す舌の体操，(2)舌ブラシでの舌の清掃と舌刺激，(3)ブラッシング後に頬の膨らましを意識した30秒間の含嗽の3種類を指導した．口腔機能向上プログラム前後の口腔機能測定結果を比較するとともに，GOHAIスコアの第1四分位により2群に分け，それぞれの群における比較も行った．その結果，口腔機能向上プログラム介入後には咀嚼能力で有意な改善が認められた．また，GOHAIスコアの低値群においては，咀嚼能力だけではなく，舌左右運動の速さ，オーラルディアドコキネシス/ka/にも改善が認められた．以上の結果より，高齢者のQOLの維持・向上には，口腔機能が関連しており，口腔機能向上のための継続したアプローチの重要性が示唆された．","subitem_description_type":"Abstract"}]},"item_6_description_5":{"attribute_name":"内容記述","attribute_value_mlt":[{"subitem_description":"学位の種類:
