glial tauopathy (GGT) は2013 年に疾患概念が提唱された，新たな4 リピートタウオパチーである．組織学的にはグリア細胞内への小球状 (globular) のリン酸化タウ陽性構造物 (Globular glial inclusions : GGIs) の存在を特徴とし，オリゴデンドロサイトではGlobular oligodendroglial inclusions (GOIs)，アストロサイトでは Globular astrocytic inclusions (GAIs) と呼ばれる．GGT は，GGIs を主体とするタウ陽性構造の出現分布と中枢神経組織の変性部位から，３つのサブタイプに分けられている．それらは臨床症状に対応し，前頭側頭葉主体の病変分布を取り前頭側頭型認知症を呈するType I, 運動野と錐体路を主体とし運動ニューロン徴候を呈するType II, そしてType I とII の複合型であるType III の３タイプである．しかし，こうしたサブタイプについては未だ確立されたものではない．そこで本研究では，GGT Type III ３例とType II ２例を対象とし，サブタイプ間の特徴について検討した．特にGAIs と神経細胞内封⼊体 (Neuronal cytoplasmic inclusions: NCIs) の形態に着目して，GGIs の定量解析，共焦点顕微鏡を用いた三次元構造解析，電子顕微鏡像による超微形態観察を行い，サブタイプ間で比較検討した．GGIs の定量解析では，GAIs の出現量はType II に比べType III で顕著に多かった．三次元構造解析では，GAIs の形態がType 間で異なることを示した．すなわち，Type II のGAIs は，小型で多数の小球状構造からなり，アストロサイトの突起遠位にまで広がっていた．一方，Type III のGAIs はゴツゴツした少数の小球状構造が胞体近位部に形成され，直径もType II より小さい傾向を示した．これらの形態的特徴の違いから，GAIs の形成過程が異なっている可能性が示唆された．NCIs には，３つの特徴的形態：即ち(1)Diffuse granular, (2)Thick cord-like, (3)Round/horseshoe-shaped NCIs を観察した．Thick cord-like NCIs はType II, Type III の両者で認めた．Round/horseshoe-shaped NCIsはType III においてのみ観察された．これらのGAIs とNCIs は，タウ陽性構造物の形態によってサブタイプが判別可能であることを示し，またGGT
