NBI 内視鏡観察による血管形態に基づいた表在型食道扁平上皮癌深達度評価の分類（IPCL分類） がある．この分類において，内視鏡治療の適応となる深達度pT1a-MM/pT1b-SM1 と推定される血管形態を示す癌に，外科治療が第一選択であるpT1b-SM2 癌が含まれており，深達度診断能は十分でない．本研究の目的は，観察因子を追加することで，拡大NBI 内視鏡による表在型扁平上皮癌の深達度診断能を向上させることである． 【対象と方法】2010年から2014年までに当院で内視鏡的粘膜下層剥離術により切除された食道癌367病変のうち，拡大NBI 内視鏡にてIPCL-V3 血管領域を有する51病変（51症例）を対象とした． IPCL-V3 領域のサイズと病理組織学的所見（癌の深達度，粘膜筋板への水平浸潤幅，分化度，浸潤様式）の関係を解析した． 【結果】 pT1b-SM2 癌におけるIPCL-V3 領域サイズは， pT1a-LPM 癌， pT1a-MM/pT1b-SM1癌のそれと比較して有意に大きかった． pT1b-SM2 癌の 73% （11/15）がIPCL-V3 領域サイズ10 mm以上であったのに対し， pT1a-MM/pT1b-SM1 癌の6% （2/31）のみがIPCL-V3領域サイズ10 mm以上であった． 【結論】IPCL-V3 領域サイズが10 mm以上である癌は，深達度pT1b-SM2 である可能性が高い．血管形態の質に加えて，その領域性（サイズ）を評価することにより，拡大NBI
