し，咀嚼・嚥下や発音機能に障害を生じる。発音において鼻からの息漏れは発話の明瞭さを低下させるため，患者の QOL に大きく影響する。口腔と鼻腔を隔てる顎義歯による封鎖性は、その機能回復に不可欠である。しかこれまでに上顎欠損患者における顎義歯の封鎖性を客観的かつ定量的に評価した報告はない。そこで本研究では、顎義歯の封鎖効果を検証すことを目的として，超小型大気圧センサを用いて上顎顎義歯装着患者における発音時の口腔内圧を評価するとともに，口腔内圧と音圧との関係について検討した。\\n【方法】被験者は、上顎腫瘍切除術後に顎義歯を装着した患者7名，対照として顎口腔に異常のない若年健者者 15名とした。測定姿勢は頭部を固定しない座位とした。超小型デジタ ル大気圧センサを義歯用安定剤にて口蓋部に装着し，口腔内圧を測定した。同時に、被験者の正面に普通騒音計を設置し発音時の音圧を測定した。タスクは /pa/ 発音とし各被験者に対 して 10回ずつ行った。\\n【結果および考察】健常者群にける /pa/ 発音時 には，平均 1. 5 kPa の最大口腔内圧が観察 された。上顎欠損患者群では，顎義歯非装着時の最大口腔内圧は平均 0.9 kPa と口腔内圧の変化をほとんど認めず，呼気が鼻腔へ漏出したためと考えられたが， 顎義歯装着により平 均 2.0 kPa まで有意に回復した．上顎欠損患者群の最大音圧は，顎義歯の有無による差を認めず， 同様に健常者群とも違いは認められなかった。口腔内圧は音圧の大きさに影響を受けており，健常者群では音圧と口腔内圧の間に中等度から強い相関が認められた。一方で， 上顎欠損患者群においても相関関係が認められたが，顎義歯装着時の相関係数や回帰式の傾 きには個人差が認められ，それらをもとに顎義歯の封鎖性を評価できる可能性が示唆され た。\\n【結論】発音時の音圧は口腔内圧と相関しており，顎義歯は上顎欠損患者の口腔内圧形成に寄与していた。
