年1 月から2012 年12 月末までに発生した人身事故292 件について，病院への救急車による搬送の状況を中心に調査を行い，救急医療の現状と問題点について検討した．交通事故292 件中，救急搬送された件数は182 件 (搬送率62%) であった．負傷者数は463 人 (軽傷者371 人，重傷者81 人，死者11 人) であり，救急搬送された負傷者は309 人 (搬送率67%) (軽傷者228 人，重傷者70 人，死者11 人)であった．事故発生時刻は，8～17 時に比較的多く発生し，21～23 時，2 時～4時は少ない傾向が認められた．事故現場から病院に搬送されるまでに要した時間は平均20 分，距離は平均15.4km であった．また事故発生現場は関越道ほぼ全域に見られ，負傷の程度とも明らかな関連性は認められなかった．救急搬送された負傷者309 人のうち，重傷者及び死者が81 人と約1/4 を占めていたが，関越自動車道が整備されている中越及び魚沼地区では，高度の救命救急医療が可能な医療機関は，長岡赤十字病院，立川綜合病院，長岡中央総合病院と長岡市に偏在している．重篤な負傷者は，受傷後1 時間以内に手術が行われるか否かが生命予後を決定する重要な因子であるが，前述の3 病院に搬送された事例において，交通事故の覚知から病院に搬送されるまでに要した時間が1 時間を超えたものが64 人中19 人，32%存在していた．2015 年6 月に開院した魚沼基幹病院が救急搬送にもたらす効果を明らかにするため，関越自動車道の各キロポスト区間における最短搬送時間と距離を算出し，魚沼基幹病院開院前と同院開院後での変化をシミュレーションした．その結果，上り線では平均搬送時間が27 分，搬送距離が26.6km 短縮され (p<0.05)，下り線では平均搬送時間が24 分，搬送距離が25.3km 短縮されることが判明した (p<0.05)．さらに上り線及び下り線がそれぞれ交差している国道及び県道に高速自動車道から直線の救急車専用退出路を設置したと仮定して，1 キロポスト毎に搬送時間を検討した．その結果，上り線では平均搬送時間が3 分短縮され (p<0.05)，下り線では搬送時間が1 分短縮された (p<0.05)．2016 年秋頃に2 機目のドクターヘリが長岡赤十字病院に導入されることから，前述の長岡市の3 病院に搬送された64 人についてその効果を検証した．その結果，平均搬送時間が19 分短縮されることが判明した(p<0.05)．しかし，ドクターヘリには，夜間運航や高速道路上に直接離着陸できない等の問題があることから，消防防災ヘリとの連携，高速道路上やサービスエリアへの離着陸の検討が必要である．さらにドクターカーの適切な運用や高速道路からの救急車専用退出路の整備等も必要である．救急搬送に関わる諸機関がこれまで以上に連携して，救急搬送を取り巻く環境及びシステムを改善・構築し，さらなる人命救助に繋げる必要があると考える．","subitem_description_type":"Abstract"}]},"item_6_description_5":{"attribute_name":"内容記述","attribute_value_mlt":[{"subitem_description":"学位の種類:
