infants"}]},"item_6_biblio_info_6":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2016-03-23","bibliographicIssueDateType":"Issued"},"bibliographic_titles":[{}]}]},"item_6_date_granted_51":{"attribute_name":"学位授与年月日","attribute_value_mlt":[{"subitem_dategranted":"2016-03-23"}]},"item_6_degree_grantor_49":{"attribute_name":"学位授与機関","attribute_value_mlt":[{"subitem_degreegrantor":[{"subitem_degreegrantor_name":"新潟大学"}]}]},"item_6_degree_name_48":{"attribute_name":"学位名","attribute_value_mlt":[{"subitem_degreename":"博士(医学)"}]},"item_6_description_4":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"【緒言】低出生体重児は周産期環境などの影響により,成長後に高血圧を発症し易い.一方で,低出生体重児は腎尿細管機能や副腎機能の未熟性により,新生児期にナトリウム(Na)欠乏に陥り易い.Naは循環血液量や浸透圧,血圧の恒常性維持に不可欠であり,その調整には副腎ホルモンが中心的な役割を果たしている.我々は,低出生体重児の新生児期Na欠乏による副腎機能への負担が,将来の血圧上昇に繋がると考えており,本研究では新生児期の副腎機能を評価し,関連因子について検討した.【対象と方法】2010年11月から2013年3月まで新潟大学医歯学総合病院総合周産期母子医療センターNICUに入院した極低出生体重児(very low birth weight;VLBW,出生体重<1500g)を対象とした.生後2週間,1カ月時に血漿Na濃度,血漿クレアチニン濃度,尿中Na濃度(UNa),尿中Cre濃度(UCre)を測定した.液体クロマトグラフィータンデムマス法(LC-MS/MS)を用いて1カ月時の尿中副腎ホルモン(dehydroepiandrosterone;DHEA,アルドステロン,コルチゾール,コルチゾン)濃度(urinary adrenal hormone concentration;UAHC)を測定し,周産期背景やNaバランスとの関連を検討した.【結果】対象はVLBW35名(女児57.1%)で在胎期間29.97±3.14週(平均±標準偏差),出生体重1094[888-1306]g(中央値[四分位])であった.1カ月時のUAHCは尿中DHEA/Cre 3.13[1.19–7.47]μg/gCre,尿中アルドステロン/Cre 7.72[3.72–11.73]μg/gCre,尿中コルチゾール/Cre 8.80[5.03–17.32]μg/gCre,尿中コルチゾン/Cre 366.88[207.32–471.52]μg/gCreであった.1カ月時のUNa/Creと全てのUAHCに正の相関を認め,出生体重と尿中DHEA/Cre(r=-0.344,p=0.043),尿中アルドステロン/Cre(r=-0.443,p=0.008)に負の相関を認めた.【結論】LC-MS/MSを用いたVLBWの生後1カ月時のUAHCを報告した.新生児期にNa欠乏に陥り易いVLBWでは副腎機能に負担がかかり,特に出生体重が小さい児でその負担が強いことが示唆された.","subitem_description_type":"Abstract"}]},"item_6_description_5":{"attribute_name":"内容記述","attribute_value_mlt":[{"subitem_description":"学位の種類:
