double strand break: 以下DSB）が，(1)UC粘膜で生じているかどうか，(2)UC粘膜におけるDSBと炎症の活動性との関連，(3)UC粘膜におけるDSBとUC罹患期間との関連，を検討した．ホルマリン固定全大腸炎型UC外科切除例104例（UC群）の直腸またはS状結腸を対象とした．正常対照は，炎症性腸疾患を合併しない大腸癌外科切除例50例（非IBD群）の直腸またはS状結腸の非腫瘍粘膜とした．UC群，非IBD群の代表切片パラフィンブロックから3μm厚連続切片2枚を作成し，HE染色とDSBのマーカーであるγH2AXに対する免疫染色を行った．UCの炎症時相は，寛解期，慢性活動期，活動期に分類した．γH2AXの発現状態は，検索切片内の総陰窩数に対するγH2AX陽性陰窩の割合（γH2AX crypt index: 以下γH2AX-CI）として評価した．(1)UC群は非IBD群に比べγH2AX-CIが有意に高値であった（中央値で11.0% vs. 0%, P<0.001）．(2)UCの炎症時相別のγH2AX-CIは，寛解期：中央値4.4%，慢性活動期：中央値13.7%，活動期：中央値15.3%であり，寛解期と活動期との間には有意差があった(P<0.001)．(3)炎症時相別にみたγH2AX-CIとUC罹患年数との間には有意な相関はなかった．これらのことから，UC粘膜では正常大腸粘膜に比べ有意に高頻度にDSBが生じており，更にDSBの頻度は炎症の活動性と関連していたことから，DSBはUCの炎症性発癌に関連しているものと推定された．一方，DSBとUC罹患年数との間には有意な相関がなかったことから，活動性炎症で生じたDSBは必ずしも罹患年数に応じて蓄積されて行く訳ではなく，DNA損傷応答（DNA damage response: 以下DDR）により修復されることが示唆された．すなわち，UCの炎症性発癌には活動性炎症によるDSBが必要条件ではあるものの，DSBに対するDDRの破綻が非可逆的な発癌過程への進行には必須であり，炎症性発癌メカニズムの解明にはDSBとDDRを併せた検討が必要と考えられた．","subitem_description_type":"Abstract"}]},"item_6_description_5":{"attribute_name":"内容記述","attribute_value_mlt":[{"subitem_description":"学位の種類:
