colitis：以下UC)の重篤な合併症として、大腸癌の発生がある。UCに発生する大腸癌はdysplasia と呼ばれる粘膜内腫瘍を前癌病変とすることから、その早期発見と治療のため、dysplasia の内視鏡的発見と生検による診断を目的としたサーベイランスが行われてきた。dysplasia の多くは平坦もしくは丈の低い隆起で周囲との境界が不明瞭であり、内視鏡的認識が困難なことから、サーベイランスでは多数個のランダム生検が推奨されてきた。しかし近年、色素拡大内視鏡観察がdysplasia の発見に有用であり、色素拡大内視鏡と狙撃生検を組み合わせたサーベイランスが推奨されつつある。しかし、これまで色素拡大内視鏡で発見されたdysplasia の多くは隆起であり、平坦型dysplasia の色素内視鏡拡大像については検討されていない。本研究では、外科切除UC 大腸粘膜にみられた平坦型dysplasia５例を対象として、色素実体顕微鏡下で観察される微細粘膜模様（pit pattern）について検討した。pit pattern は工藤らの分類に準じ、I型、II型、IIIL 型、IIIs 型、IV型、V型に分類した。平坦型dysplasia のパラフィンブロックから3μm 切片でHE 染色、Ki-67免疫染色、p53免疫染色を施行した。HE染色標本でlow-grade dysplasia (LGD)とhigh-grade dysplasia (HGD) の診断を行い、対応する実体顕微鏡写真との一対一対応を行い、平坦型dysplasia のpit pattern 分類を行った。平坦型dysplasia は5例全例がLGDであり、4 例はI型pitを呈し、１例はIIIs型とIV型pit とが混在していた。I型pit を呈する４例では、Ki-67免疫染色で同定される増殖帯は腺管中層以深に存在し、p53 蛋白過剰発現も腺管中層以深に認められた。これらの病変では腺管の構造異常を呈さないものが多かった。IIIs 型とIV型pit が混在する1 例では、増殖帯は腺管中層～全長に存在し、p53 蛋白過剰発現も腺管のほぼ全長にわたって認められた。これらの病変では腺管の構造異常がみられた。以上のことから、UC の平坦型dysplasia は規則正しいI型円形pit からなる微細表面性状を呈するものが多く、通常の大腸の腫瘍性病変の診断に有用なpit pattern診断により診断することは困難と考えられた。しかし逆に、長期の慢性持続性炎症により荒廃したUC 大腸粘膜では非腫瘍性粘膜が不規則なpit pattern を示す可能性があることから、不規則なpit pattern 領域に認められる規則正しい円形pit pattern が平坦型dysplasia
