carcinoma (以下MP)の生物学的悪性度、および低分化型腺癌との予後比較を行い、大腸癌における独立した組織型としてのMP の臨床病理学的意義について検討した。外科切除大腸癌516例（うち予後追跡調査が行われたものは509例）を対象とした。MPの病理組織診断は、HE染色標本、D2-40免疫染色標本、PAS粘液染色標本で行い、HE染色標本で空隙内に微小乳頭状の癌胞巣が認められ、かつ癌のリンパ管侵襲や粘液結節内に存在する癌胞巣であることが否定されたもののみをMPとした。対物40倍１視野以上の領域でMPを認めた症例をMP成分ありとした。低分化型腺癌成分に関しても、同様の基準でその有無を判定した。大腸癌516例中68例（13.2%）にMP成分の併存を認めた。MP成分併存癌(MP癌)は非併存癌（non-MP癌）に比べ、脈管侵襲陽性率、リンパ節転移陽性率、遠隔転移陽性率いずれも有意に高頻度で、TNM stageの病期進行度もStage III-IV の頻度が有意に高かった。予後比較でも、MP癌はnon-MP癌に比べ有意に予後不良であった。低分化型腺癌成分併存癌との予後比較では、MP成分のみが存在する群、低分化型腺癌成分のみが存在する群、両成分が存在する群の間で予後に有意差はなかった。これらのことから、大腸のMP は生物学的悪性度が高い癌の組織成分と考えられたが、低分化型腺癌とは予後で代表される生物学的悪性度には差がないことから、MPを大腸癌の独立した組織型として分類・診断することの臨床病理学的意義は乏しいと考えられた。","subitem_description_type":"Abstract"}]},"item_6_description_5":{"attribute_name":"内容記述","attribute_value_mlt":[{"subitem_description":"学位の種類:
