UC合併大腸癌(Ulcerative Colitis-Associated Colorectal Carcinoma : UC-CRC)では,高頻度に胃型への粘液形質変化を認めるが,慢性持続性炎症粘膜からの発癌過程における背景粘膜の粘液形質変化に関しては十分に解明されていない.本研究の目的は, UC合併大腸癌背景粘膜における胃型への粘液形質変化を解析し,胃型への粘液形質変化は発癌高危険群を選別するための指標となるかを明らかにすることである.【方法】外科切除が施行された全大腸炎型UC-CRC9例を対象とした.外科切除が施行された非担癌全大腸炎型UCのうち, UC-CRC群に年齢, UC羅患期間を極力合致させた9例を抽出しUC対照群とした. UC対照群とUC-CRC群における非腫瘍性直腸粘膜における胃腺窩腺型粘液形質であるMUC5ACに対する免疫組織化学を施行し, MUC5AC発現様式および発現頻度を検討した.MUC5AC発現様式はsporadic type (散在性発現)とdiffuse type (びまん性発現)とに分類した.【結果】 UC-CRC群において検索した4882陰窩中1161陰窩(23.8%)がsporadic typeのMUC5AC発現を示し,UC対照群における3769陰窩中408陰窩(10.8%)と比較してUC-CRC群で有意にsporadic typeのMUC5AC発現頻度が高かった(p<0.001). UC-CRC群において検索した4882陰窩中206陰窩(4.2%)がdiffuse typeのMUC5AC発現を示し, UC対照群における3769陰窩中7陰窩(0.2%)と比較してUC-CRC群で有意にdiffuse typeのMUC5AC発現頻度が高かった(P <0.001).UC-CRC群では9例(100%), UC対照群では8例(89%)がsporadic typeのMUC5AC発現を認めた(p>0.999).
