pneumonia"}]},"item_6_biblio_info_6":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2014-03-24","bibliographicIssueDateType":"Issued"},"bibliographic_titles":[{}]}]},"item_6_date_granted_51":{"attribute_name":"学位授与年月日","attribute_value_mlt":[{"subitem_dategranted":"2014-03-24"}]},"item_6_degree_grantor_49":{"attribute_name":"学位授与機関","attribute_value_mlt":[{"subitem_degreegrantor":[{"subitem_degreegrantor_name":"新潟大学"}]}]},"item_6_degree_name_48":{"attribute_name":"学位名","attribute_value_mlt":[{"subitem_degreename":"博士（医学）"}]},"item_6_description_4":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"慢性閉塞性肺疾患（COPD）患者では、増悪時においても呼吸リハビリテーション(呼吸リハ)が運動耐容能とQOLを改善することが認められ、超急性期を脱した後に早期に運動療法を開始すべきと考えられている。一方、臨床の場ではCOPD以外の間質性肺炎（IP）等の慢性呼吸疾患患者の増悪時に対しても呼吸リハを実施することがあるが、それらの有用性に関する情報は不十分である。急速に進行するIPに対する呼吸リハの有用性、適切な開始時期・適応が検討課題である。対象は、2007年7月から2012年1月まで当院に急速に進行するIPの診断で入院し、呼吸リハを行った19症例である。動脈血ガス分析、CRP、LDH、KL-6、入院から呼吸リハ開始までの期間、実施期間、Barthel index (BI)による日常生活動作（ADL）を評価し、呼吸リハの早期介入群と非早期介入群、呼吸リハ開始時の吸入酸素量にて低流量群と高流量群、呼吸リハ終了時のBI低得点群とBI高得点群に分けて検討した。入院から呼吸リハ開始までの期間は41.1±37.8日で、呼吸リハ前後のBIは60.9±23.5点から73.8±21.2点に有意に改善した(p＝0.049)。BIの改善に、呼吸リハ介入時期、酸素吸入量による有意差は認めなかった。BI高得点群は呼吸リハ開始時点ではより若年で、歩行のみ有意に高く、ほかに有意差を認めなかったが、呼吸リハ終了時点では車いすからベッドへの移動、トイレ動作、入浴、歩行、階段昇降が有意に高くなり、ADLが改善していた。急速に進行するIPにおいても積極的な呼吸リハは、ADLに寄与すると考えられた。急速に進行するIPに対し、急性期や高流量酸素吸入下でも呼吸リハの適応はあり、70歳以下で歩行可能であれば効果的に実施でき、ADL改善により多くの効果をもたらす可能性が高い。","subitem_description_type":"Abstract"}]},"item_6_description_5":{"attribute_name":"内容記述","attribute_value_mlt":[{"subitem_description":"学位の種類:
