MSはミエリン・オリゴデンドロサイトに発現する何らかの分子が自己抗原と推測されている. NMOSDはアストロサイト足突起にある水チャネル分子アクアポリン4 (aquaporin-4: AQP4) が自己抗原である. MSの原因は未だ不明のままだが,NMOSDは液性免疫による補体介在性のアストロサイト傷害が中核の病態である.しかしAQP4抗体が,血液脳関門を超えてアストロサイトのAQP4分子を認識する機序は不明である.MSでは,細胞浸潤機構の重要分子としてmelanoma cell adhesion molecule(MCAM)の関与が唱えられている. MCAMは,免疫グロブリンスーパーファミリーに属する細胞接着分子で,T細胞と血管内皮細胞の接着,T細胞の血管外遊走に関与し,MCAM陽性T細胞はインターロイキン(interleukin:IL)-17を産生し,IL-17が脳血管関門の破綻を引き起こすと考えられている.実際MSの再発時の脳脊髄液中や活動性病変ではMCAM陽性T細胞が増加し,MS動物モデルである実験的自己免疫性脳脊髄炎ではMCAM陽性IL-17産生T細胞の中枢神経系への浸潤が示されている.私は,NMOSD においても,MCAM陽性IL-17産生T細胞が,IL-17を介し血液脳関門を傷害し,AQP4抗体の侵人を惹起し,その結果としてアストロサイトが傷害されるとの仮説をたて,NMOSD脊髄病変におけるT細胞とMCAM発現を免疫組織化学的に検討した.対象は,NMOSD剖検例7例と,非中枢神経疾患剖検例5例の脊髄組織とした.NMOSDの38病変にてMCAM陽性単核球の有無,MCAM陽性T細胞の密度(単位面積当たりの個数),T細胞中のMCAM陽性細胞の割合,MCAM陽性血管の有無を解析した.病変部位は,既報に基づき,initial lesions(IL), early active lesions (EA), late active lesions (LA), inactive lesions (IA) に分類した. MCAM陽性単核球は,IL/EA の86%, LAの75%, IA の22% に認めた.
