B型急性大動脈解離により，腹部臓器ならびに下肢の虚血を呈した症例に対して，早期血流再開目的からaxillobifemoral bypassが施行され，良好な結果が報告されている．しかし，順行性体血流と人工血管バイパスによる下肢からの逆行性灌流による競合が，術後に人工血管の開存性や残存解離腔へ及ぼす影響，さらに最終的な大動脈リモデリングヘ及ぼす影響は不明である．今回，axillobifemoral bypass術後8年目に，意識消失を呈して再搬送された症例を経験したので，長期経過中の大動脈リモデリング状況を含めて報告する．症例は76歳男性で，突然の腰背部激痛と両下肢脱力を認め，緊急搬送となった．胸腹部CTにて，大動脈遠位弓部から両側腸骨動脈分岐部にかけての解離を認め，Stanford B型急性大動脈解離の診断で降圧療法を開始した．しかし，進行する偽腔拡大から，下肢の間歇性跛行と腎機能低下を認め，axillobifemoral bypassを施行した．術後経過中，右前側胸部に血清腫を形成したため，バイパス術後9ヶ月目に，蛇行した人工血管と腫瘤の除去を行った．その際のCTにて，良好なバイパス血流，および横隔膜レベルでの偽腔の縮小と血栓化を認めた．今回，バイパス術後8年経過した時点で，意識消失発作を生じて緊急搬送となった．入院時の胸部CTでは，遠位弓部から腎動脈下腹部大動脈にかけて，偽腔の著明拡大と真腔の狭小化を認めた．また，axillofemoral bypassは閉塞していたが，腹部分枝は真腔から造影され，描出に問題はなかった．臓器虚血を合併したStanford B型大動脈解離は手術適応であるが，急性期での人工血管置換術は，侵襲が大きく，未だに術後成績が不良である．近年，医療器材と技術の発展から，かかる致死的合併症を有する急性B型大動脈解離例に対して，ステントグラフ卜留置術が第一選択となりつつある．しかし，技術発展が十分でなかった時期には，低侵襲の緊急処置としてaxillobifemoral bypassが一般的であったが，術後長期遠隔期でのバイパス開存性や残存解離腔へ及ぼす影響は不明である．今回，腎・下肢虚血を合併したStanford B型解離に対して施行したaxillobifemoral bypass術後の長期遠隔期に，偶然にもCT検査ができた症例を経験した．その所見から，axillobifemoral bypasにより，一時的に狭小化した真腔の拡大傾向を認めたものの，長期遠隔期には，下肢からの逆行性灌流を形成していたaxillobifemoral bypassは閉塞し，相対的に増加した順行性体血流が，開存したentryに作用して，より一層の偽腔拡大を招来した可能性が推測された．以上から，遠隔期の良好な大動脈リモデリングを獲得するためにも，腎・下肢の虚血が解除された段階で，可及的早期にentry閉鎖が肝要と考える．","subitem_description_type":"Abstract"}]},"item_7_full_name_3":{"attribute_name":"著者別名","attribute_value_mlt":[{"nameIdentifiers":[{"nameIdentifier":"176157","nameIdentifierScheme":"WEKO"}],"names":[{"name":"Moro,
