The eruptive process of the mandibular first molars was investigated during the period from alveolareruption to preoraleruption. The subjects were 14 patients whose second primary molars were prematurely lost in one side and remained on the other side. Longitudinal oblique cephalograms were used to estimate the eruptive process of the first molars on both the lost and remained sides. The effect of the M-loop appliance on the guidance for eruption of the first molars was also observed on the lost side. The results were as follows. 1) The first molars on the remained side showed three patterns of positional changes in the horizontal aspect during the period from the alveolareruption to the preoraleruption. The molars drifted mesially, distally or did not drift. 2) In the case of mesial drift, the space of the molar region was narrow at the alveolareruption. The first molars erupted rapidly followed by the marked changes of the axial inclination of the crown to the occlusal line. At the same time, the space of the molar region became enlarged. 3) In the case of distal drift, the space of the molar region was large at the alveolareruption. The first molars hardly changed the axial inclination of the crown to the occlusal line, and the space of the molar region was scarecely enlarged. 4) The vertical movement for eruption of the first molars was observed to be normal on the lost side.
