Thirty two teeth in 27 periodontitis patients were used for this study. After completion of the initial preparation, probing pocket depth (PPD) and clinical attachment level (CAL) were measured, as determined the use of GTR and baseline data. Following gingival flap elevation, scaling, root planing and removal of granulation tissue on the periodontal defect sites, GTR membrane was placed and sutured. Gingival flap was repositioned and sutured tightly, so as to cover the membrane as completely as possible. After 4 to 6 weeks, the membrane was removed again and clinical assessment was repeated at 6 months following surgery and compared with the baseline. The results were as follows: 1. Class Ⅱ furcation involvements (11sites) showed a significant vertical and horizontal PPD reduction (mean±SD ; vertical : 1.5±1.4mm, horizontal : 2.3±1.0mm ; p<0.01). They also showed CAL gain at six months after GTR (0.9±1.6mm). 2. Three-wall intrabony defects (21sites) disclosed a significant vertical PPD reduction (3.1±1.7mm ; p<0.01) and CAL gain (2.2±1.4mm ; p<0.01). 3. The use of GTR produced significantly more CAL gain than flap surgery alone. These results suggest that GTR using a ePTFE membrane is very effective on treatment of both class Ⅱ furcation involvement and three-wall intrabony defect.","subitem_description_type":"Abstract"},{"subitem_description":"成人性歯周炎と診断された27名の患者32歯(F2級根分岐部病変11歯、 3壁性垂直性骨欠損21歯)に、初期治療終了後、ePTFE (expanded polytetrafluoroethylene)メンプレン(Gore-Tex(R)、GTR membrane)を用いた組織再生誘導法(GTR法)を行った。初期治療後の評価値:プロービングポケット深さ(PPD)、及び臨床的アタッチメントレベル(CAL)をベースラインとした。歯肉弁を剥離、スケーリング・ルートプレーニソグを可及的に行ったのち、メンプレンを試適、縫合固定し、歯肉弁を戻し縫合した。術後4週から6週後にメンプレンを除去、術後6ヶ月目に再評価を行った。その結果、1. F2級根分岐部病変を有する11歯のPPDは、術後6ヶ月目でべ-スライン時と比べて垂直的に(平均1.5±1.4mm)、水平的にも(平均2.3±1.0mm)有意な減少が認められた。また、臨床的アタッチメントレベル(CAL)は、平均0.9±1.6mmの獲得が認められた。2. 3壁性垂直性骨欠損を有する21歯のPPDはベースライン時に比べて有意に減少し(平均3.1±1.7mm)、CALは平均2.2±1.4mmと有意な獲得がみられた。3.
