 note = {This study was designed to assess the effectiveness of simplified reading method of panoramic tomography (PT) applied to volunteers consisted of undergraduate dental students and young trainee dentists. Eleven PTs including eight with malignant lesion of the maxillary sinus and the adjacent tissue were given to the volunteers who were requested to read films within two minutes and to record their findings. Then the volunteers were divided into two groups. Group A repeated to read the same films again without providing any new information. Group B repeated to read the same films again within two minutes after a schematized check point list was given. Group A did not show any slgnificant improvement, whereas Group B showed definite improvement in pointing out abnormal findings which could not be detected in the previous film reading. In routine daily practice it is very difficult for general dentists to read radiograms in detail on each patient. .In these circumstances there is a strong tendency to miss unfamiliar structures that may result in overlooking malignant lesions. The present study clearly showed that paying attention to some important lines on PTs that can easily provided by a simplified schematic check list facilitates to find malignant lesions which are usually unfamiliar to general dentists., 【目的】本報告の｢その1｣で示したような症例に対する不十分な画像診断がどのような理由によってなされたものであるか検証し、またパノラマ写真に対する一般歯科医や歯科学生の関心の動向、読影上の問題点を探り、パノラマ写真に対するより確実な読影手法を確立するための方向性を検討する【対象と方法】上顎部及びその周辺に悪性病変のあるパノラマ写真を用い、学生や研修医を対象として2回の読影実験をおこなった。2回目の実験では、一部の集団を除き、パノラマ撮影の原理や第1報で示した｢特に重要な構造｣を記載した簡易資料を与えた｡【結果】読影実験の第1回目では、異常所見が明らかな場合でも検出率は低く、第2回目では、簡易資料を与えなかったグループを除いて成績は向上した｡しかし各構造別に成績を見ると、｢洞底｣などの異常についてはよく検出されたものの、｢硬口蓋｣｢頬骨突起｣｢翼状突起｣などについては検出率は低く、簡易資料が与えられた第2回目の読影においても、特に後2者に対する成績の向上はわずかにとどまった。【考察】日常的な関心の高さが｢洞底｣の異常に関する検出を容易にしていると考えれば、症状の有無に関わらず、日常撮影されるパノラマ写真において、｢その1｣であげたような構造を常に確認すべきである。それは潜在的な悪性病変の検出のみならず、これらの構造に関する描出像の差違や変異の把握につながり、ひいては上顎部及びその周辺のパノラマ診断能力の向上にも寄与すると考えられる。},
