Hyperthermia combined with radiochemotherapy was attempted for the treatment of an oropharynx cancer with an RLN and bilateral cervical lymph nodes metastases, and resulted in a complete response. A 65-year old man was referred to our clinic complaining of pain in the oropharyngeal region. MRI imaging revealed an oropharyngeal tumor mass lesion and some metastases to an ipsilateral RLN and bilateral superior internal jugular lymph nodes (SIJLN). Pathologically, it was a well-differenciated squamous cell carcinoma, classified as stage 4 according to the UICC classification. We treated this patient with a combination of hyperthermia, using a 2,450-MHz microwave heating system, irradiation and chemotherapy. The tumor temperature inside the bilateral SIJLN was maintained at over 43℃ for about 40 minutes. Hyperthermia was performed 8 times (twice a week), combined with 60Co irradiation (2Gy/day, total70Gy), CDDP (10mg/day, total 60mg) and 5FU (125mg/day, total 5,625mg) administered through intra-arterial injection. The tumor size dramatically decreased, and finally disappeared 6 months later judging based on MRI. The patient died of pneumonia 1 year 10 months later without any reccurence around the oropharyngeal area and regional lymph nodes. The present case suggests that hyperthermia combined with radiochemotherapy is an effective strategy for the advanced head and neck cansers with retropharyngeal lymph node metastases.","subitem_description_type":"Abstract"},{"subitem_description":"外側咽頭後リンパ節(いわゆるRouviereリンパ節:RLN)に転移をきたした頭頚部癌の制御は極めて困難である｡今回、RLNおよび両側頚部リンパ節に転移をきたした中咽頭癌に対し、放射線化学療法を併用した温熱療法を施行し、著効を得たので報告する｡症例は65歳、男性、口峡咽頭部の痛みを主訴に当科を受診｡MRIでは同部に腫瘍陰影、片側のRLNおよび両側上内探頚リンパ節(SIJLN)に転移を認めた｡病理組織学的に高分化型扁平上皮癌と診断され、UICC分類ではStage4である｡本例に放射線療法と化学療法を併用し、2450MHzのマイクロ波空間放射型加温装置を用いた温熱療法を施行することとした｡両側のSIJLNにおける腫瘍内温度は43℃以上約40分間維持された｡温熱療法は8回(週2回)施行し、60Co照射(2Gy/日、計70Gy)後、CDDP(10mg/日、計60mg)と5FU
