This randomized, double-blind, placebo-controlled, split-mouth study was designed to compare the clinical and radiographical effects of EMD treatment to that of placebo-controlled treatment for infrabony defects. Sixteen patients were included in this clinical trial, each of whom had one or two pairs of infrabony defects located contralaterally in the same arch. Thirty-six infrabony defects were randomly assigned treatment with flap surgery plus EMD or flap surgery plus placebo. At baseline and at the 6 month follow-up evaluation visit, clinical and radiographic measurements were determined. Data were statistically analyzed using the Wilcoxon-signed ranks test (a=0.05). At the 6 month evaluation visit, bleeding on probing for the EMD group was less 0.11±0.32, compared to the placebo group, 0.72±0.46(P<O.01). Pocket probing depth reduction was greater in the EMD group (2.83±1.17mm) compared to the placebo group (1.94±0.73mm) (p<0.05). Mean values for clinical attachment gain in the EMD and the placebo groups were 1.50±0.99mm and 0.61±0.70mm, respectively (p<O.05). Radiographic bone density gain was greater in the EMD (11.4±12.9%) compared to the placebo group (-2.29±13.5%) (p<0.01). Treatment with flap surgery and EMD, compared to flap surgery with placebo, produced a significantly more favorable clinical improvement in infrabony periodontal designおよび二重盲検法により検討した。成人性歯周炎と診断され歯周基本治療後に同一顎の適切に離れた同名歯歯間部に6mm以上の歯周ポケット深さ(PPD)とアタッチメントロス(CAL)を有し、かつⅩ線写真で骨内欠損が認められた16名18対を対象とした｡両部位に対して全層歯肉弁を形成･剥離し、病巣部掻爬完了後に歯根面を36%正リン酸で15秒間エッチングを行い、EMD投与群およびplacebo投与群を無作為に割り付けた｡術後6か月目に臨床評価、Ⅹ線評価を行った。Bleeding on probingはEMD群がplacebo群と比較して有意に低かった｡PPD減少量およびCAL獲得量はEMD群で有意に優れていた(△PPD:EMD群:2.83±1.17mm vs placebo群:1.94±0.73mm, p<0.05, △CAL:EMD群:1.50±0.99mm vs placebo群:0.61±0.70mm,p<0.05）。骨密度変化率(△RBD)についてはEMD群で11.4±12.9％、placebo群で-2.29±13.5%、p<0.01であった。
