Preoperative polysomnography and lateral cephalograms were compared with those recorded three months postoperatively. In the present study, the criteria for being classified as a responder was a postoperative apnea hypopnea index (AHI) < 20 with at least a 50% reduction of preoperative AHI. Overall success rate of UPP was 47% but rose to 67% if patients with severe OSAS were excluded. The mean values of preoperative AHI, body mass index, facial axis, distance from palatal plane to hyoid and postoperative length of soft palate in nine responders were smaller than those in 10 non-responders and the mean values of preoperative lowest SpO2 and length of soft palate in responders were larger than those in non-responders. These results suggest that UPP is a reliable treatment for nonobese patients with mild or moderate OSAS, who have been found to have the site of obstruction located at the oropharynx.","subitem_description_type":"Abstract"},{"subitem_description":"当科において口蓋垂軟口蓋形成術(UPP)を施行した閉塞型睡眠時無呼吸症候群(OSAS)19症例についてその治療効果を検討した。対象患者の重症度は、AHIが20未満の軽症が4名、20から40の中等症が5名、40以上の重症が10名であった。治療の効果の評価には、術前と術後3か月時の終夜睡眠ポリソムノグラフイー(PSG)ならびに側方頭部Ⅹ線規格写真を用いた｡本研究では、術後3か月時のAHIが20未満で術前の値の50%以下に達したものをUPPが有効であったと判定した｡その結果、OSASに対するUPPの有効率は、47%であったが、重症例を除くと67%に有効であった｡有効群(9例)では無効群(10例)に比較して、術前のBMI、AHI、PPH、Facial axisおよび術後の軟口蓋長の値が小さく、術前の軟口蓋長とlowest
