Total case number of general anesthesia tended to increase and reached over 450 cases per year while case number of local anesthesia remained unchanged for these years. There was no gender difference among the patients. Patients ranged from one month old to ninety-four years old. The number of patients ranged between fifteen years old and twenty-four years old were the common and it tended to increase during this decade. The operations related to the cleft lip and palate, or orthognathic surgery were common operations, 37.7%, or 21.1% of all, respectively. The number of orthognathic surgery increased these five years to the largest extent, but in contrast the cases of general anesthesia for dental treatment were quite very few, only 40 case of all. Induction of anesthesia was performed with a rapid induction and propofol became to be used as an induction drug for recent years. Oral or nasal endotracheal intubations were performed in 61.9%, or 36.7%, respectively, but nasal intubation tended to increase during this decade. Sevoflurane was used for slow induction and maintenance of general anesthesia. The number of case within two hours to four hours complied 58.5% of all and the number of these short hour operation tended to increase. The most common pre-operative complication was hyper tension, and the postoperative complications were pain in throat and nausea and/or vomiting. There were no serious complication like as cardiac arrest. These trends suggest that the number of general anesthesia and local anesthesia with any complications expect to increase furthermore, resulting a need for resolving insufficient manpower., 平成2年4月の新潟大学歯学部附属病院歯科麻酔科開設以来12年間に、中央手術室で施行した4650例の全身管理症例をレトロスペクティブに検討した。 1)全身麻酔症例数は4008例、局所麻酔症例数は642例であり、全身麻酔症例数には年々増加する傾向があった。 2)症例数に性差はなかった。 3)症例の年齢分布は、生後1か月から94歳11か月にわたり、15~24歳の年齢層で年々増加する傾向があった。 4)疾患別症例数では、平成12年度以降で顎変形症手術の割合が顕著に増加していた。 5)麻酔導入法では、静脈麻酔薬による急速導入法が74.4%を占め、使用麻酔薬ではプロポフォールの使用が増加していた。 6)気道管理法では経鼻挿管の占める割合が増加していた。 7)術中の麻薬性鎮痛薬の使用は年々増加していた。 8)麻酔時間は2時間から4時間の短い手術時間の症例数が増加する傾向にあった。 9)術前合併症では高血圧症が最も多かった。 10)術後合併症には、咽頭痛・嘔気・嘔吐が多く、心停止などの重篤な合併症はなかった。 今後も中央手術室での全身管理症例は増加することが予測され、マンパワーの強化に取り組む必要があると思われた。}, pages = {37--43}, title = {新潟大学歯学部附属病院中央手術室における最近12年間の全身管理症例の動向}, volume = {33}, year = {2003} }
