The age of the patients ranged from 62 to 74 years (mean, 67.7 years). The diameters of six lesions ranged from 43 to 74mm (mean, 58.5mm). Hyperthermia was induced once or twice a week for a total of 7.8 times on average, in combination with cisplatin-based chemotherapy (mean, 105mg) following irradiation (mean, 58.7 Gy). We have three kinds of heating system, a microwave system, a radiofrequency (RF) system, and an RF interstitial system, and one of them was applied for each lesion. Of the six lesions treated, two (33.3%) showed complete response (CR); three (50.0%), partial response (PR); and one (16.7%), no change (NC). Therefore, the response rate was 83.3%. Pain was alleviated in all the patients. As follow-up treatment, two of the three patients who showed PR underwent an operation. Complete or almost complete disappearance of viable tumor cells was confirmed histopathologically. The five-year cumulative local control and survival rates in all the patients were 66.7%. In conclusion, thermochemoradiotherapy is an effective strategy for patients even with advanced or recurrent carcinoma of the lower gingiva.","subitem_description_type":"Abstract"},{"subitem_description":"当科では1990年以後、切除不能な下顎歯肉癌の進行例3例または術後再発例3例の計6例に対し、温熱化学放射線療法を施行したので報告した｡患者の年齢は62歳から74歳、平均67.7歳である｡6病巣の大きさ(長径)は43mmから74mm、平均58.5mmである｡温熱療法は3方法(マイクロ波加温(2例)、RF誘電加温(3例)、RF組織内加温(1例))を用いた｡加温回数は4から17回、平均7.8回で、シスプラチン(平均105mg)多剤化学療法と放射線療法(平均58.7Gy)を同時併用した｡6病巣の一次効果はComplete response(CR)が2例(33.3%)、Partial response(PR)が3例(50.0%)、No change (NC)が1例(16.7%)で、奏効率は83.3%であった｡全例除痛効果がみられた｡後続治療としてPRとなった3例中2例は切除可能となった｡切除物の病理組織学的所見では生存癌細胞なしと一郎の生存癌細胞ありが各1例であった｡5年累積局所制御率および生存率は66.7%であった｡温熱化学放射線療法は進行･再発下顎歯肉癌に対する有力なtreatment
