 note = {There is no consensus on whether or not patients under antithrombotic treatment who are to undergo oral surgical procedures would benefit from drug cessation, which could reduce transoperative hemorrhagic mishaps. While some believe that a short-term interruption of the medication would not lead to thrombus formation, others suggest a more cautious approach without iscontinuation. As many recent reports have shown that oral surgery can be successfully performed on patients undergoing ntithrombotic therapy, we have treated such patients without drug cessation at Sado Municipal Ryotsu Hospital sinceJune 2004. In this report, we summarize our experience on treating unexpected transoperative hemorrhage with local hemostasis under a controlled environment in a total of 14 cases. In 2 cases (14.3%) we had to manage postoperative bleeding, hemostasis was generally easily achieved, and no thrombotic crisis occurred throughout the surgical treatment. Concerning bleeding control, absorbable hemostatic materials applied locally and splints covering the extraction sockets were shown to be extremely effective. And furthermore, the management under short-term admission was very convenient for even if sudden postoperative bleeding. oncomitantly, We performed a questionnaire survey on the management of hemorrhage about oral surgery in medical staff from Sado Island. Surprisingly, only 37.5% of the surveyed personnel knew that teeth extractions could be safely performed on patients under antithrombotic treatment without therapy interruption. The results of our survey unveiled the need for educating Sado's medical community on the convenience of treating such patients in a safer way, with suitable local measures and a controlled environment., 抗血栓療法患者の歯科観血処置に際して、抗血栓薬を休薬するか否かの判断は各施設で異なる。短期間の休薬であれば血栓発症の可能性は低いとする意見や周術期の出血を危惧して休薬を勧める意見など様々であるが、最近では、非休薬下でも安全な歯科観血処置が可能であるとする報告も散見されるようになっている。当科でも2004年6月から原則非休薬で対応することとし、術直後の局所止血と術後数日の予期せぬ出血に対する管理体制の整備に重点を置いている。今回、抗血栓薬を休薬せずに歯科観血処置を行った14症例について、周術期の経過を調査したところ、術後出血は2例(14.3%)に認められ、いずれも止血は容易であった。局所止血には、止血材料と止血シーネが有効で、短期入院下で管理することにより、予期せぬ出血に対しても迅速かつ適切な処置が可能であった。さらに、周術期を通して血栓発症を疑う所見はなく、総じて安全な処置が可能であった。また、周辺地域の医療スタッフを対象にアンケート調査を行ったので、その結果を踏まえて、今後は関係機関との連携に努めたいと考えている。},
