{"created":"2021-03-01T06:30:56.125761+00:00","id":25463,"links":{},"metadata":{"_buckets":{"deposit":"89dcda99-60b4-4dec-8cb3-fd4f68c4d533"},"_deposit":{"id":"25463","owners":[],"pid":{"revision_id":0,"type":"depid","value":"25463"},"status":"published"},"_oai":{"id":"oai:niigata-u.repo.nii.ac.jp:00025463","sets":["432:1291:1292:1299","453:456"]},"item_7_alternative_title_1":{"attribute_name":"その他のタイトル","attribute_value_mlt":[{"subitem_alternative_title":"A case of adenosquamous carcinoma of the tongue."}]},"item_7_biblio_info_6":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2009-12","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"2","bibliographicPageEnd":"163","bibliographicPageStart":"159","bibliographicVolumeNumber":"39","bibliographic_titles":[{"bibliographic_title":"新潟歯学会雑誌"},{"bibliographic_title":"新潟歯学会雑誌","bibliographic_titleLang":"en"}]}]},"item_7_description_4":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"腺扁平上皮癌は病理学的に扁平上皮癌と腺癌の両方の性格を有する悪性腫瘍である。腺扁平上皮癌はまれに鼻腔・口腔粘膜などの顎口腔領域に発生する。私たちは舌に発生した腺扁平上皮癌の1例を経験したので報告した。患者は25歳男性。智歯抜歯の依頼を受け当科を受診。初診時の口腔内診察にて舌に潰瘍があることを指摘した。初診時，右側舌縁に8×3mmの潰瘍を認め，その周囲に硬結を伴う堤防状の隆起を13×11mmの範囲で認めた。MRI では右側舌縁に腫瘍と思われる病変が認められた。しかし，頸部リンパ節転移，遠隔転移は認められなかった。初診時の細胞診はclassIV であり，臨床診断を舌癌の疑いとした。腫瘍は小さく初期であり，生検により転移をする可能性もあったため生検は施行せず，全身麻酔下に舌腫瘍切除術と智歯抜歯を施行した。切除標本所見では扁平上皮癌の性格を有する部位と，腺癌の性格を有する部位が認められた。腺癌様の病変部位ではアルシアンブルー・PAS 染色にて陽性の所見を示していた。よって最終的な病理組織学的診断は，腺扁平上皮癌とした。術後7年8か月経過した現在も再発・転移無く予後は良好である。","subitem_description_type":"Abstract"},{"subitem_description":"Adenosquamous carcinoma is a malignant tumor with hitolosical findings of both squamos cell carcinoma and adenocarcinoma. It rarely occurs in the nasal and oral mucosa of the maxillofacial region. We reported a case of adenosquamous carcinoma of the tongue. A 25-years-old man visited our clinic because of the extraction of a third molar. We examined his oral cavity for the extraction of a third molar and found a lingual ulcer. On initial examination, there was a 8x3mm ulcer which tender and surrounded by induration (13x11mm). Magnetic resonance imaging (MRI) revealed a solid mass in the tongue of right side. However there was no evidence of Lymph-node metastasis in the neck, distant metastasis. The tumor was suspected a neoplasm of the tongue. Initial cytologic specimen showed class IV. Biopsy was not performed, because the mass is early stage carcinoma and examination may cause metastasis. We resected the tumor and extracted a third molar under general anesthesia. Histological examination of the surgical specimen showed both squamous cell carcinoma and adenocarcinoma. Adenocarcinoma-like lesion of the tumor showed positive reaction to PAS and Alcian blue staining, and the tumor was finally diagnosed as adenosquamous carcinoma.
