The procedure was occasionally combined with hemodialysis, when both hepatic and renal failure occurred. We discussed the clinical features and treatment of 27 patients with hepatic and renal failure. Four of 6 patients with hepatic failure followed by renal failure had pre-existing liver cirrhosis. On the other hand, 3 of 4 patients with renal failure followed by hepatic failure had operation of valvular heart disease. However, among 8 patients showing hepatic and renal failure at the same time, 2 revealed valvular heart disease, 3 malignancy, 2 infectious disease and 1 unknown cause respectively. Concerning the method of plasmapheresis, approximately 30 units of fresh frozen plasma was required to get sufficient removal of bilirubin. We treated renal failure with continuous arterio-venous hemofiltration, which could be applied for even the patients with circulatory disturbance.
