Following experimental study was done with Japanese monkeys to investigate the validity of this idea. 1. One cm long short bridge graft in FDP tendon within the flexor sheath healed well and clinical application of this technique was considered to be justified. Adhesion at the suture sites at both stumps of the grafted tendon was much slighter in specimens with complete closure of the sheath than in those with partial resection of the sheath, examined 4 weeks and 3 months after the grafting. 2. In specimens in which resected portion of flexor sheath was substituted with a free graft of extensor retinaculum, adhesion at both suture sites of the short tendon graft was as slight as in specimen with complete closure of the sheath by replacing the partially raised flap of the sheath. 3. The reasons why adhesion at both suture sites of short tendon graft was comparatively slight in specimens with complete closure either with temporarily raised sheath flap or with a graft of extensor retinaculum were considered as follows: it prevented invasion of newly proliferated connective tissue into the suture sites and also it facilitated diffusion of the synovial fluid into the suture sites. 4. The results described above led to the conclusion that the flexor sheath should better be closed after short bridge tendon graft, although its advantage was not proved in clinical cases with direct tendon repair within the flexor sheath. 5. In experimental specimens with 4 cm long tendon graft whose distal suture was within the flexor sheath and whose proximal suture was outside of and proximal to the flexor sheath, adhesion of the grafted tendon within the sheath was much lighter than outside of it, although adhesion at both suture site was essentially of the same grade. 6. Even in specimens with adhesion around the grafted tendon, which occasionally took place, little adhesion was seen around the proper FDP tendon being located distal and proximal to the tendon graft. Therefore, localized tenolysis might suffice to release the adhesion. 7. Early postoperative motion gave rise to no good result in this experimental study causing disruption of the suture site, probably because control of motion was very difficult as in clinical cases of children. 8.
