Although its activity is elevated in pathological conditions such as multiple myeloma and periodontal disease, little has been focused on its action in vivo. The present study was undertaken to assess the effect of OAF on human bone remodeling with monocytic bone mineral (^<45>Ca-hydroxyapatite crystals) resorption assay system in osteoporotic patients and healthy volunteers. Iliac bone biopsies from 17 subjects were also performed to determine its role in vivo. After these investigations, following results have been obtained. 1. There was no significat difference of OAF activity in osteoporotic patients in comparison to that in healthy volunteers. 2. Increase of OAF activity was found in patients with rheumatoid arthritis. 3. OAF activity has significant correlation with the parameters related to bone formation which are based in bone histomorphometry. These results lead to the conclusion that OAF activity varies among diseases but is not likely to be elevated in osteoporotic patients except in rheumatoid arthritis.
