PBC cases were composed of 10 cases of stage 1, one case of stage 2 and 3 cases of stage 3 by Scheuer's classification. Viral hepatitis cases were composed of 11 cases of acute viral hepatitis (type A hepatitis : 2, type, B hepatitis : 4, type non-A, non-B hepatitis : 5) and 10 cases of chronic hepatitis (type B hepatitis : 1, type non-A, non-B hepatitis : 9). In PBC cases, varied changes of bile duct epithelia were observed as follows ; degenerative and destructive changes, disarrangement, swelling of mitochondria with disappearance of cristae, increased intermediate filaments, bleb formation and desquamation. In viral hepatitis cases, these ultrastructural changes of biliary epithelial cells were found less frequently and in a lower grade than those in PBC cases. While thickened, multilayered, irregular and partially disappeared basement membranes were observed in both cases, osmium positive substances in thickened basement membrane were found in PBC cases and not in viral hepatitis cases. It was demonstrated that the appearance of osmium positive substances in the basement membrane of biliary epithelial cells was the disparity between PBC and viral hepatitis.
