For this purpose, consecutive 90 autopsy and 134 surgical materials were examined by mirror-image paraffin-embedded tissues. ECM was frequently found in these areas, and its aggregation-pattern was classified into four types^2 Nodular type (Type 1) (2/196 in the minor papilla, 1/165 in the major papilla), Scattered type (Type 2) (45/196, 8/165), Orifice type (Type 3) (8/196, 0/165) and Islet-like type (Type 4) (18/196, 0/165). The endocrine cells of these ECMs were usually immunoreactive only for somatostatin and/or pancreatic polypeptide except for those of type 4. The type 4 was regarded to be atrophic or regenerating islet of Langerhans, and was finally excluded from ECM. By a tentative criteria, distinct carcinoid (type 1), neoplastic ECMs, and non-neoplastic ECMs were found in 1.0%, 8.2% and 16.8% of 196 cases in the minor papilla, and 0.6%, 1.2% and 3.6% of 165 cases in the major papilla respectively. In the duodenal papillae, especially in the minor duodenal papilla, carcinoid tumor and ECM seem to be present more frequently than generally thought.
