The object was 25 patients with mucosal carcinoma and 48 with submucosal carcinoma. The total number of superficial carcinomas which included tumors independently found in the cases with advanced carcinoma was 131 mucosal carcinomas and 52 submucosal carcinomas. Patients with mucosal carcinoma showed better prognosis (100% of five year survival rate) than those with submucosal carcinoma (76.5%). Lymphatic permeation, venous invasion, and nodal metastasis of mucosal carcinomas were positive in 4%, 4%, and 0%, respectively, and the rates were significantly lower than those of submucosal carcinoma ; 64.6%, 68.8%, and 30.4%, respectively. Macroscopically, 30 (90.9%) of 33 tumors with elevation over 1mm (0- I type) in the deepest cancer-invasion area were submucosal carcinomas. Tumors with depression more than 0.5mm (0- III type) and those with both elevation and depression (mixed type) invaded the submucosal layer. 128 (90.1%) of other 142 tumors (0- II type) were mucosal carcinomas. 63 (98.4%) of 64 tumors less than 10mm in size were mucosal carcinomas. On the other hand, 26 (92.6%)of 28 tumors in which the size were 40mm or more were submucosal carcinomas. Striped surface pattern of the tumor indicated mucosal carcinoma, 55 (91.7%) of 60 tumors with smooth・flat surface pattern and 45 (73.8%) of 61 tumors with granular, fine granular or coarse surface pattern in the deepest cancer-invasion area were mucosal carcinoma. Nodular and subepithelial surface pattern indicated submucosal carcinoma. The area, color and peripheral pattern of tumors were also available for distinguishing mucosal carcinoma from submucosal carcinoma. It is possible to differentiate mucosal carcinoma from submucosal carcinoma macroscopically.
