Donor hearts were separated into four groups: Gr.I, transplanted immediately affter 4℃ saline perfusion (n=15), Gr. II, transplanted immediately after 4℃ GIK solution perfusion (n=15), Gr, III, 90 minutes preservation in non-oxygenated 4℃ GIK solution (n=11), Gr. IV, 90 minutes preservation in oxygenated 4℃ GIK solution (n=11). In Gr. I and Gr. II, transplanted hearts were removed at varying intervals after surgery (7 days, 14 days, 30 days) and analyzed for morphological change. In Gr. III and Gr. IV, three transplanted hearts were removed at 1 hour and 24 hours and analyzed by means of electron microscopy. The remaining five hearts were removed at day 7 and analyzed for morphological change. We made a quantitative evaluation of myocardial fibrotic change (%fibrosis) by the point-counting method. Electron microscopically the Gr. III hearts showed more severe ischemic change compared with Gr. IV at 24 hours. In Gr. III and Gr. IV, the LV % fibrosis means were 25±3.4%(subendocardial layer 25±2.5%, midmyocardial layer 21±3.4%, epicardial layer 23.4±4.2%) and 14±2.7% (subendocardial layer 12±3.0%, midmyocardial layer 16±4.0%, epicardial layer 14±2.1%) respectively. The % fibrosis of the LV and the subendocardial layer was significantly lower in Gr. VI (P<0.05).
