 note = {It is possible and indispensable that severely impaired neurologic patients are treated by comprehensive community medical care system in Niigata city now. We have treated about 400 patients by the home care system since 1978 in Shinrakuen Hospital. The hospital treatment for such patients should be teamwork. The team members are neurologists, neurosurgeons, rehabilitation staff, nurses, public health nurses and medical social workers. After discharge, these severely handicapped patients are treated continuously with a wide variety of professionals such as primary care doctors, hospital doctors, public health nurses, home visiting nurses, home helpers belonging to city office and volunteers. The coordinator among these members is the public health nurse in Shinrakuen Hospital. The monthly meeting of Niigata liaison conference for intractable disease started in 1989 and effects as a coordinator.},
