virtual simulation : IVS)法により作成した手術計画を手術顕微鏡と連動させたニューロナビゲーションシステムに融合し，実際の摘出術の手術支援法として確立したので報告する．【材料と方法】大型の頭蓋底髄膜腫13症例に対し，放射線画像データから作成した3-D CG画像をもとにIVS法を用いて術前シミュレーションを行った．まず骨開窓や脳の牽引により腫瘍への到達路を作成，次いでこの到達路内での視点の移動による見え方の変化や周辺構造との関係などから，大きな腫瘍の摘出可能域を順次コンパートメント化して行った．IVSで作成したデータは，手術室でのニューロナビゲーションシステムのデータに融合し，このシステムと手術顕微鏡を連動させることで，実際の術野を3-D CG画像やスライス画像の連動により確認しながら腫瘍摘出を遂行した．【結果】全症例において3-D CGデータの作成とIVS法による術前シミュレーションが有効に行われた．腫瘍は3～6個のコンパートメントに分割された．手術中はナビゲーション画面上に3-D CG画像，連動させた顕微鏡手術画像，および各種軸断面画像を表出し，腫瘍摘出を進めながら3-D CG画像の腫瘍も削除し，残存部を確認しながら手術を施行した．予測していなかった大量の出血があった2症例を除いて，11症例の摘出術は手術計画と一致した．【考察と結論】大型頭蓋底髄膜腫をコンパートメント化し摘出する手術計画を，我々独自のIVS法から顕微鏡連動ナビゲーションシステムへの融合を通して視覚的に遂行することを可能とし，「IVS誘導手術」と呼べる高難度頭蓋底腫瘍手術に対する新たな手術支援法を確立することができた．","subitem_description_language":"ja","subitem_description_type":"Abstract"}]},"item_6_description_53":{"attribute_name":"学位記番号","attribute_value_mlt":[{"subitem_description":"新大院博(医)第1034号","subitem_description_language":"ja","subitem_description_type":"Other"}]},"item_6_dissertation_number_52":{"attribute_name":"学位授与番号","attribute_value_mlt":[{"subitem_dissertationnumber":"甲第4964号"}]},"item_access_right":{"attribute_name":"アクセス権","attribute_value_mlt":[{"subitem_access_right":"open
