{"created":"2022-04-04T06:59:31.017344+00:00","id":2000318,"links":{},"metadata":{"_buckets":{"deposit":"c22d575c-22c8-4ef3-a377-3bcba36ff458"},"_deposit":{"id":"2000318","owners":[1],"pid":{"revision_id":0,"type":"depid","value":"2000318"},"status":"published"},"_oai":{"id":"oai:niigata-u.repo.nii.ac.jp:02000318","sets":["453:455","471:561:562"]},"author_link":[],"control_number":"2000318","item_6_date_granted_51":{"attribute_name":"学位授与年月日","attribute_value_mlt":[{"subitem_dategranted":"2021-03-23"}]},"item_6_degree_grantor_49":{"attribute_name":"学位授与機関","attribute_value_mlt":[{"subitem_degreegrantor":[{"subitem_degreegrantor_language":"ja","subitem_degreegrantor_name":"新潟大学"},{"subitem_degreegrantor_language":"en","subitem_degreegrantor_name":"Niigata University"}],"subitem_degreegrantor_identifier":[{"subitem_degreegrantor_identifier_name":"13101","subitem_degreegrantor_identifier_scheme":"kakenhi"}]}]},"item_6_degree_name_48":{"attribute_name":"学位名","attribute_value_mlt":[{"subitem_degreename":"博士（医学）","subitem_degreename_language":"ja"}]},"item_6_description_4":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"目的・方法：HIV感染症の長期療養にあたり，メンタルヘルスの管理は重要な課題である。本研究では外来通院症例を対象とした横断研究により，睡眠障害の有無をピッツバーグ睡眠質問票(Pittsburgh Sleep Quality Index ; PSQI)日本語版で評価し，HIV症例における睡眠障害の合併頻度とその詳細について明らかにするとともに，うつ病，認知機能，その他の臨床背景との関連についても調査した。結果：対象は18歳以上の成人60例で，年齢の中央値は44.5歳，85.0％が男性であった。PSQIによる判定の結果，睡眠障害の合併は33例(55.0％)と高頻度で認められ，質問票の個々の項目としては睡眠時間の問題が最頻であった。またPatient Health Questionnaire(PHQ)-9日本語版による評価では，27例(45.0％)がうつ病ありと判定され，睡眠障害とうつ病の併存は22例(36.7％)で認められた。さらに30症例でHIV関連神経認知障害(HIV-associated neurocognitive disorder ; HAND)のスクリーニング検査を行った結果，7例(23.3％)がHANDであると判定され，睡眠障害とHANDの併存は6例(20.0％)で認められた。結論：PSQIスコアを軸とする共分散構造分析の結果は，睡眠障害に最も影響が大きい項目はうつ症状で，両者の密接な関連が示唆された。またBody Mass Index(BMI)値も優位な関連が認められ，潜在的に閉塞性睡眠時無呼吸が関与している可能性も考えられた。睡眠障害への対応にあたり，こうした要因への適切な介入が重要と考えられた。","subitem_description_language":"ja","subitem_description_type":"Abstract"},{"subitem_description":"Objective・Methods : Management of mental health is an important issue in the long-term treatment of HIV infection. In this study, we conducted a cross-sectional study of HIV-positive patients to evaluate sleep disorders using the Japanese version of the Pittsburgh Sleep Quality Index (PSQI). In addition, we also investigated the relationship between depression, cognitive function, and other clinical backgrounds with sleep disorders. Results : The subjects were 60 adults 18 years of age and older, with a median age of 44.5 years and 85.0% male. Among them, 33 cases (55.0%) were diagnosed as sleep disorders using PSQI, and the problem of sleep time was the most common answers in the questionnaire. According to the Japanese version of Patient Health Questionnaire(PHQ)-9, 27 cases (45.0%) had depressive symptoms, and 22 cases (36.7%) had both sleep disorders and depression. Additionally, screening tests for HIV-associated neurocognitive disorder (HAND) were performed in 30 cases, and 7 cases (23.3%) were evaluated as HAND, and 6 of 7 cases had complications of sleep disorders. Conclusions : The results of covariance structure analysis centered on the PSQI score indicated that depressive symptoms had the greatest impact on sleep disorders. Body Mass Index also had a significant association with sleep disorders, suggesting the potential complication of obstructive sleep apnea.
