{"created":"2021-12-08T06:38:49.909435+00:00","id":2000129,"links":{},"metadata":{"_buckets":{"deposit":"3f2e6f5f-74c6-4f65-8d92-f9d94b4a5cc0"},"_deposit":{"id":"2000129","owners":[1],"pid":{"revision_id":0,"type":"depid","value":"2000129"},"status":"published"},"_oai":{"id":"oai:niigata-u.repo.nii.ac.jp:02000129","sets":["453:456","471:537:538:1638944799941"]},"author_link":[],"item_1627361970403":{"attribute_name":"出版タイプ","attribute_value_mlt":[{"subitem_version_resource":"http://purl.org/coar/version/c_970fb48d4fbd8a85","subitem_version_type":"VoR"}]},"item_7_biblio_info_6":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2020-04","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"4","bibliographicPageEnd":"117","bibliographicPageStart":"113","bibliographicVolumeNumber":"134","bibliographic_titles":[{"bibliographic_title":"新潟医学会雑誌","bibliographic_titleLang":"ja"}]}]},"item_7_description_4":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"2017年度の医学部定員は過去最多となったが，新潟県は都道府県別にみた医療施設に従事する人口10万対外科専門性資格保持医師数はワースト2位であり，将来必要とされる外科医師数を達成するためには年間40人の外科医を育成しなければならない．この度，日本専門医機構により新専門医制度が確立され，2018年度から外科新専門医制度は開始されたが，専攻医の募集定員は全国204プログラムで計2,061人でありそのうち採用されたのは805人であった．新潟県では2プログラムで計8名を採用した．新潟大学外科専門医プログラムでは2018年4月から6名の専攻医が，2019年4月から8名の専攻医が研修を開始した．卒後臨床研修2年間と専攻医としての3年間(1～3年次)規定の手術数を経験する必要があるが，すでに1年次の段階で各分野別の必須手術経験数のほとんどを経験することができた．手術総数350例および術者数120例以外は，すべて新潟大学外科専門医プログラム1年次で経験できる見込みであり，2年次および3年次は大学病院以外の関連病院で術者として手術を経験することになる．サブスペシャルティ領域がすでに決まっているのであれば，2年次3年次の研修はサブスペシャルティ領域を意識してその領域を専門的に研修できる．我々のプログラムにおける連携施設は新潟県でのhigh volume centerが中心となったが，これはプログラムの整備基準により手術症例数や指導医数に応じて，募集できる専攻医数が決まってしまうことによる．これにより集約化がさらに進んでいくものと考えられるが，新潟県は広い医療圏を持つため集約できない地域も存在するので，そういった地域での医療をどのように行っていくかが喫緊の課題である．また，いわゆる「地域枠」あるいは新潟県の修学賓金を受けていた卒後医師は，地域での指定勤務が義務付けられるのであるが，そのために専門医取得が不利にならないように留意して制度づくりを行った．これまで新潟大学外科学教室は「3科1つ屋根の下に」という理念の下，教育指導を行ってきた．今後とも3科で協力体制をとりながら，教育指導にあたっていきたい．","subitem_description_language":"ja","subitem_description_type":"Abstract"}]},"item_7_publisher_7":{"attribute_name":"出版者","attribute_value_mlt":[{"subitem_publisher":"新潟医学会","subitem_publisher_language":"ja"}]},"item_7_source_id_11":{"attribute_name":"書誌レコードID","attribute_value_mlt":[{"subitem_source_identifier":"AN00182415","subitem_source_identifier_type":"NCID"}]},"item_7_source_id_9":{"attribute_name":"ISSN","attribute_value_mlt":[{"subitem_source_identifier":"0029-0440","subitem_source_identifier_type":"PISSN"}]},"item_access_right":{"attribute_name":"アクセス権","attribute_value_mlt":[{"subitem_access_right":"open access","subitem_access_right_uri":"http://purl.org/coar/access_right/c_abf2"}]},"item_creator":{"attribute_name":"著者","attribute_type":"creator","attribute_value_mlt":[{"creatorNames":[{"creatorName":"滝沢, 一泰","creatorNameLang":"ja"},{"creatorName":"Takizawa, Kazuyasu","creatorNameLang":"en"}]},{"creatorNames":[{"creatorName":"亀山, 仁史","creatorNameLang":"ja"},{"creatorName":"Kameyama, Hitoshi","creatorNameLang":"en"}]},{"creatorNames":[{"creatorName":"若井, 俊文","creatorNameLang":"ja"},{"creatorName":"Wakai, Toshifumi","creatorNameLang":"en"}]},{"creatorNames":[{"creatorName":"土田, 正則","creatorNameLang":"ja"},{"creatorName":"Tsuchida, Masanori","creatorNameLang":"en"}]},{"creatorNames":[{"creatorName":"木下, 義晶","creatorNameLang":"ja"},{"creatorName":"Kinoshita.
