A 56-year-old male, had uncontrolled diabetes mellitus that was treated with humulin-N. An abdominal CT scan that was done to detect the origin of the diabetes mellitus showed a low density tumor, 4×5 cm in size, with strongly enhanced margins, at the pancreas tail. An ERCP study showed a defect in the pancreas field, and celiac arterial angiography revealed a hypervascular tumor. The serum somatostatin level was elevated (74.3 pg/ml). The somatostatin level of the splenic vein, which was taken intraoperativly, had a high titer (276.9 pg/ml). Following a diagnosis of pancreatic endocrine tumor, and pancreatic tail resection and splenectomy, the pathological specimen showed no malignant tumor changes; immunohistological staining demonstrated somatostatin positive cells in the tumor margin.
